– Thai nationality
– Live or work in Samut Sakhon Province
– never been infected with coronavirus before
– never vaccinated against any type of coronavirus before
– work or residents Has a name in the house registration of Samut Sakhon Province
Documents to be presented
– Original ID card to show
– Photocopy of ID card
– proof of residence or working in Samut Sakhon Province
* If there is no name in the Samut Sakhon house registration
Documents to be presented
– Photocopy of ID card along with one of the following evidences:
– Work certificate or photocopy of employee identification card with the address of the establishment in Samut Sakhon Province, or
– Certificate of residence in Samut Sakhon Province with The host/Kumnan/Ban headman/Sor.Sor.Sor./Sor.Sor.Sor./Government officials are the ones who sign for certification. along with a photocopy of the identity card of the certifier or a photocopy of the commercial registration certificate or the commercial registration certificate in Samut Sakhon Province (with the name of the person who will be injected as the owner of the business) or a photocopy of the receipt for the house fee / dormitory / rented room or a rental contract with the name of the person who will be injected in the document or a receipt for water / electricity / internet bills at home address in Samut Sakhon Province, where the name of the person who will be vaccinated on the receipt
