Form Sheet is the 85th film of Suresh who is completing different films. 2500A four sheets of various colors used by children for making curiosities were used. The 50-feet-long and 30-feet-wide film was made in two days in the swimming pool on the fifth floor of Vibe Resort in Munnar. His son Indrajith, Rakesh Pallath and Sandeep became assistants.
