Research from Mass General Brigham indicates that increasing daily step counts may significantly delay the onset of cognitive decline and the progression of Alzheimer’s disease, particularly in older adults with high levels of amyloid-beta.

The study, published in Nature Medicine, suggests that even modest increases in physical activity can slow the buildup of harmful proteins in the brain and preserve thinking skills and daily functioning.

Researchers analyzed data from 296 participants aged 50 to 90 years old as part of the Harvard Aging Brain Study. At the start of the study, all participants were cognitively unimpaired.

The team used PET brain scans to establish baseline levels of tau in tangles and amyloid-beta in plaques. Physical activity was tracked using waistband pedometers, and participants underwent annual cognitive assessments for a period ranging from two to 14 years, with an average follow-up of 9.3 years.

A subset of the participants received repeated PET scans, which allowed researchers to track how tau proteins changed over time. The data showed that higher step counts were linked to a slower buildup of these proteins in participants who had elevated baseline levels of amyloid-beta.

The role of physical activity is a recognized component of broader dementia prevention strategies. According to the CDC, nearly 45% of all dementia cases may be prevented or delayed through lifestyle changes.

Physical activity is listed as a critical habit for maintaining brain health, as it helps prevent, delay, or manage chronic diseases. The CDC recommends that adults aim for 150 minutes of physical activity each week, which averages to at least 20 minutes per day.

Other lifestyle factors that contribute to the risk of dementia include uncontrolled diabetes, high blood pressure, hearing loss, and the use of tobacco and alcohol. The CDC notes that excessive drinking can lead to brain injuries or high blood pressure, both of which increase dementia risk.

Limitations and Expert Perspectives

While the Mass General Brigham study provides specific step count benchmarks, some medical professionals caution against oversimplifying the findings. A neurologist cited by CNN stated that relying on a specific number of daily steps to prevent Alzheimer’s is too simplistic.

However, the research underscores the importance of early intervention. Dr. Jasmeer Chhatwal noted that lifestyle factors appear to impact the earliest stages of the disease, suggesting that changes made early can slow the emergence of cognitive symptoms.
