A new medication called baxdrostat has demonstrated significant success in lowering dangerously high blood pressure in patients who are resistant to standard treatments. According to results from a Phase III clinical trial, the drug reduced systolic blood pressure by nearly 10 mmHg, a reduction that can meaningfully lower the risk of kidney disease, stroke, and heart attack.

The findings were presented on August 30, 2025, at the European Society of Cardiology (ESC) Congress in Madrid and published in the New England Journal of Medicine. The international BaxHTN trial was led by Professor Bryan Williams of the UCL Institute of Cardiovascular Science and sponsored by AstraZeneca.

Trial Scope and Methodology

The BaxHTN trial involved nearly 800 patients across 214 clinics worldwide. The study specifically targeted individuals with uncontrolled or resistant hypertension—those whose blood pressure levels remain dangerously high despite the use of multiple existing medications.

Baxdrostat is administered as a tablet. In the trial, the addition of baxdrostat to a patient’s background therapy resulted in a significantly lower seated systolic blood pressure at 12 weeks compared to those receiving a placebo.

The research received support from the NIHR Biomedical Research Centre at UCLH.

Mechanism of Action

Baxdrostat works by blocking the excess of aldosterone, a hormone that causes the body to retain salt and water. This hormonal imbalance is a primary driver of uncontrolled hypertension in many patients.

By targeting this specific hormone, the treatment provides a different pharmacological approach than many standard blood pressure medications, offering a potential solution for those who do not respond to conventional therapies.

Public Health Context

Hypertension is a widespread global health issue, affecting approximately 1.3 billion people worldwide. In about half of these cases, the condition is either uncontrolled or resistant to treatment.

In the United Kingdom, approximately 14 million people live with hypertension. For those with resistant high blood pressure, there is a substantially higher risk of early death and severe cardiovascular complications.

Regulatory Status

Following the clinical trial results, AstraZeneca has submitted a New Drug Application for baxdrostat in the United States. The application has been accepted under FDA Priority Review for the treatment of patients with hard-to-control hypertension.
