The Vital Role of Blood Components
Blood isn’t simply transfused as a whole unit anymore. Modern medicine relies on specific blood components, each with a critical role. Blood plasma, for example, is essential for emergency care and is considered a “liquid gold” due to its life-saving properties. At Würzburg University Hospital, approximately 20,000 blood units are used annually – an average of 55 per day.
“On the one hand, this is the surgical area, so a lot of products are used during operations. And the other area, these are patients with cancer. This group of patients also needs a relatively large number of blood products,” explains Dr. Jürgen Kößler, medical director of clinical transfusion medicine and hemotherapy at the UKW.
Beyond plasma, platelet preparations – vital for patients with bleeding disorders – have a short shelf life of just four to five days and are costly, around 400 to 500 euros per unit. Red blood cells, crucial for oxygen transport after surgery or in cases of anemia, are sourced primarily from blood donation services like the Bavarian Red Cross. These have a slightly longer shelf life of 35 to 42 days and cost around 150 euros per unit. The process is further complicated by the need for precise blood type and antibody matching, adding to the demands on healthcare providers.
The Quest for Artificial Blood
Despite advancements in medical technology, a viable artificial blood substitute remains elusive. Researchers are exploring the possibility of producing blood components in the laboratory, using precursor cells to generate platelets and red blood cells. However, this technology is not yet sophisticated enough for widespread clinical use.
“It would of course be very good if we could also produce products in the laboratory, in vitro, so to speak. Basically, there are already precursor cells from which you can then, I would say, produce platelets or erythrocytes. But that is not yet so sophisticated that it would be sufficient to actually be able to produce blood products from it. When it comes to the coagulation factors, we have proteins that are missing in hemophiliacs. And It’s actually possible that One can… And of course it would be a great goal to continue working on this in order to ultimately be able to produce these cellular blood products, for example,” says Dr. Kößler.
A Volunteer System Under Strain
Germany’s blood donation system relies heavily on volunteers, a network that, while well-organized, is vulnerable to fluctuations in donor availability. Seasonal illnesses, like flu and colds, and even events like holidays can significantly reduce the number of eligible donors. The unpredictable nature of major accidents requiring large volumes of blood adds another layer of complexity.
“On the one hand, we have the donors who work together. And the blood donation facilities, who always try very hard to ensure that everything goes well with the donations, that it is very well organized, that nothing medically ever happens. Because that would be very bad if something happened to someone during the donation,” explains Dr. Kössler.
Patient Blood Management: A Proactive Approach
To mitigate the impact of potential shortages, hospitals like the UKW are implementing “patient blood management” strategies. This involves proactively addressing anemia before surgeries, utilizing blood-saving surgical techniques, and carefully evaluating the necessity of transfusions. These measures can reportedly save up to 20 percent of blood units without compromising patient safety.
Concerns About the Future
Despite these efforts, concerns remain about the long-term stability of the blood supply. The dismantling of blood departments at university hospitals, as seen in Saxony-Anhalt, raises fears of losing specialized expertise and increasing reliance on external sources. Proposed EU guidelines aimed at harmonizing blood supply regulations are causing concern that they could lead to increased competition and potentially compromise national self-sufficiency.
“And of course it is particularly important to ensure that we continue to have a system that we can rely on, that we are still able to supply ourselves nationally and that no undercutting competition, a market, arises where we then run the risk of not being able to provide sufficient blood products,” says Dr. Jürgen Kößler.
The current situation is not a crisis, but rather a persistent balancing act. As of January 25, 2026, reports indicated supplies were critically low, with enough for only a day and a half of need. Germany requires approximately 15,000 blood donations daily to meet demand. The Paul-Ehrlich-Institut has implemented a weekly reporting system to monitor blood supplies and facilitate timely donation appeals. The decline in donations, from 4.68 million units in 2009 to 3.43 million in 2021, underscores the need for sustained public engagement. Every donation, however small, plays a vital role in ensuring a safe and reliable blood supply for those in need.
