U.S. Sees Alarming Rise in Extreme Obesity Cases
The United States is facing a growing crisis as rates of extreme obesity, defined as a body mass index (BMI) over 60, have skyrocketed. Pennington Biomedical Research Center found a 210% increase in these cases between 2001 and 2023. This alarming trend, detailed in The Lancet: Diabetes & Endocrinology, highlights the urgent need for targeted interventions.
While overall obesity has increased by about 30% in the last two decades, the disproportionate surge in extreme cases presents unique challenges. Individuals with a BMI of 60 or higher,now estimated to number over 850,000 in the U.S., frequently enough experiance impaired mobility, increased health risks, and higher healthcare costs.
Dr. Philip Schauer, director of the Metamor Institute at Pennington Biomedical, emphasized the severity of the situation. He noted that while general obesity receives considerable attention, the extreme end of the spectrum carries a greater clinical burden. “This study is the first to categorize the growing pervasiveness of this extreme degree of obesity, but its gravity is not to be understated, as patients in this category face far greater health risks and comorbidities,” Schauer said.
To put the numbers in perspective, someone with a BMI of 30 is roughly 30 to 40 pounds overweight. A person with a BMI exceeding 60 is more than 200 pounds overweight. This level of obesity often leads to mobility issues, frequent hospital visits, and disability.
The rise in extreme obesity also coincides with the increased use of GLP-1-based medications. However, their effectiveness in individuals with a BMI over 60 remains uncertain, as clinical trials typically involve participants with lower BMIs.
The healthcare industry faces significant hurdles in treating these patients. Standard medical equipment often has weight limits that these individuals exceed, complicating imaging, anesthesia, and surgical procedures. This can result in inadequate care for a population already at high risk.

John Kirwan, executive Director of Pennington Biomedical, called for immediate action. “The rate of growth in cases of obesity with a BMI of 60 or greater sends an urgent message to healthcare providers and policymakers to develop the necessary resources and infrastructure to address the challenges posed by this condition,” Kirwan said.
