Nearly half of U.S. adults - a staggering 122.4 million people – are living with high blood pressure (BP), according to the 2025 American heart Association Statistical Update. This makes hypertension a leading preventable risk factor for heart disease, stroke, and premature death. Alarmingly,only approximately 25% of those diagnosed have their BP under control,highlighting a critical need for improved diagnosis and effective management strategies.
The consequences of uncontrolled hypertension are ample. Beyond the immediate risks of heart attack and stroke, chronic high blood pressure can lead to kidney disease, vision loss, and cognitive decline.The economic burden is also significant, with billions of dollars spent annually on healthcare costs related to hypertension and its complications.
Target: BP™ – A Collaborative Approach to Improvement
in response to this pressing public health challenge,the American Heart Association and American Medical Association (AMA) launched Target: BP™ in 2015. This national initiative aims to empower healthcare organizations to improve blood pressure control rates among their patient populations.
The initiative focuses on three key pillars:
- Leveraging Evidence-Based Frameworks: Utilizing American Heart Association science and established guidelines to help care teams organize their approach to BP management.
- Providing Support and Resources: Assisting healthcare organizations with professional education, practice tools, and quality improvement programs.
- Recognizing Achievement: Annually recognizing organizations that demonstrate commitment to improvement, adopt evidence-based practices, and achieve BP control rates of 70% or greater.
Record recognition: 2,307 Organizations Honored in 2025
This year,Target: BP™ is recognizing 2,307 healthcare organizations – a 17.5% increase from the 1,962 recognized in 2024 – for their dedication to tackling uncontrolled hypertension. These organizations span 49 states and U.S. territories, collectively serving over 38 million patients, including nearly 10.6 million individuals with hypertension.
Notably, over 40% of participating organizations are nonprofit health centers that receive federal funding, demonstrating the initiative’s commitment to reaching underserved communities. This is crucial, as disparities in hypertension control rates are well-documented, with certain racial and ethnic groups experiencing disproportionately higher rates of uncontrolled BP.
