Cannabis use is linked to a 37% higher risk of stroke, according to a massive meta-analysis led by researchers at the University of Cambridge examining data from more than 100 million people. The findings, published in the International Journal of Stroke, highlight that recreational substances once viewed primarily through the lens of addiction and overdose carry significant, measurable cardiovascular and neurological dangers.

Scope of the Cambridge Meta-Analysis on Stroke Risk

Stroke ranks as the world’s third leading cause of death and disability combined. While traditional risk factors like high blood pressure, smoking, and poor diets are well-documented, researchers increasingly focus on how commonly used recreational substances contribute to vascular events. To clarify these risks, scientists from the Department of Clinical Neurosciences at the University of Cambridge pooled data covering over 100 million individuals.

According to the published study data, cocaine and amphetamine use roughly double the overall risk of stroke. Cocaine use increased the risk by 96%, while amphetamines raised it by 122%. Cannabis use showed a statistically significant increase of about 37%. Meanwhile, researchers did not find a statistically significant association between opioid use and overall stroke risk.

Heightened Vulnerabilities in Younger Adults

When researchers restricted their evaluation to individuals younger than 55, the relationship between specific drugs and cerebrovascular events shifted sharply. Amphetamine use in this younger cohort was associated with an almost threefold rise in stroke risk, surging by 174%. Cocaine use among younger adults linked to a 97% increase, and cannabis use raised the risk by 14%.

In England and Wales alone, roughly 2.9 million adults aged 16 to 59 reported using a legal or illegal recreational drug within a one-year period. In the United States, more than half of individuals aged 12 and older have used substances such as cannabis, cocaine, or opioids at least once in their lifetimes.

Genetic Insights Point Toward Causal Pathways

To determine whether these associations represent direct causation rather than mere correlation, the Cambridge team employed a statistical method known as Mendelian randomization. This approach evaluates naturally occurring genetic variants linked to specific risk factors to test for causal relationships. The genetic evidence supported a causal link for multiple substances.

According to the genetic analysis, cocaine use disorders showed a strong connection to brain hemorrhage and cardioembolic stroke, a condition where a blood clot forms in the heart, travels to the brain, and blocks blood flow to damage tissue. Cannabis use disorders linked specifically to stroke overall, with a pronounced signal for large artery stroke. Problematic alcohol use also associated with higher risks of cardioembolic stroke and large artery stroke, while alcohol addiction increased overall stroke risk. Researchers could not apply Mendelian randomization to amphetamines due to a current lack of large-scale genetic datasets for that drug.

Biological Mechanisms Linking Drugs to Cerebrovascular Events

Scientists studying these pathways point to several physiological mechanisms that help explain why these substances trigger strokes. Sudden blood pressure spikes, blood vessel spasms and narrowing, and heart rhythm problems all destabilize the vascular system. Furthermore, cannabis use is associated with increased blood clotting, while amphetamines frequently trigger inflammation or vasculitis in blood vessels.
