Federal health authorities reported an additional 178 cases of measles across the United States this week, pushing the total number of confirmed infections to 4,588 for 2026 as of Friday, according to the Centers for Disease Control and Prevention. That nationwide tally marks more than double the total reported for 2025, with the illness actively driving at least 43 separate outbreaks.

The continued spread of the highly contagious virus unfolds alongside mounting criticism of the CDC regarding transparency. The agency has faced scrutiny as health secretary Robert F. Kennedy Jr. implements changes to federal vaccine safety infrastructure, a move that coincides with state-level data showing thousands of infections concentrated in Pennsylvania, South Carolina, and Utah.

Pennsylvania Outbreak Totals and Hospitalizations

Pennsylvania remains the epicenter of the national surge, accounting for 1,338 confirmed cases, according to the Pennsylvania Department of Health. State authorities report that 255 individuals have required hospitalization during the outbreak, while state figures cited by Reuters indicate measles-related hospitalizations rose from 247 to 251 between Oct. 19 and Oct. 21.

State health officials have confirmed five measles-associated deaths in Pennsylvania and are actively investigating a sixth. The Pennsylvania Department of Health noted that the state’s healthcare providers administered more than 56,000 doses of the measles, mumps, and rubella vaccine in September, followed by more than 39,000 doses so far this month, well above the typical baseline of 25,000 monthly doses.

National Surveillance and Discrepancies in Mortality Counts

Beyond Pennsylvania, large outbreaks have taken hold in South Carolina and Utah, both logging more than 500 cases, while official national numbers continue to face questions over completeness. Measles stands out as one of the most contagious diseases known to medicine, and common early symptoms such as a runny nose, cough, fever, and rash often fail to immediately prompt a doctor’s visit, leaving the official CDC count as an undercount.

Discrepancies have also emerged between state and federal mortality tracking. While the CDC acknowledges three measles deaths nationwide, federal officials have refused to specify the location of the third fatality, leaving it unclear whether the agency is counting a death recorded in Pennsylvania or elsewhere. According to Reuters, Kennedy asked senior CDC officials in September to remove a measles death from the official tally after questioning whether the disease actually contributed to the death.

Federal Policy Changes and Vaccine Safety Reporting

The Department of Health and Human Services announced structural modifications to vaccine injury reporting systems, proposing a rule that will allow physicians to bill for reporting suspected vaccine injuries. The Vaccine Adverse Event Reporting system has historically served researchers tracking rare safety issues, though it has also been cited by anti-vaccine advocates as a source of unverified claims.

Before Kennedy took office, an expert panel reviewed individual vaccine adjustments to weigh benefits against risks. Kennedy subsequently dismissed all 17 members of that board and appointed ideologically aligned replacements, though a federal court blocked those appointments in March. Public health officials emphasize that the MMR vaccine remains approximately 97% effective against infection after two doses, with unvaccinated or unknown individuals accounting for 96% of cases nationally.

Unresolved Public Health Questions and Transmission Chains

Health authorities have not yet established that the multi-state outbreaks are contained, with active transmission continuing across multiple regions. Questions remain regarding the final resolution of disputed mortality tallies at the federal level and whether upcoming epidemiological surveillance measures will successfully interrupt active chains of transmission in Pennsylvania and other affected states.
