Durvalumab Plus Chemotherapy Sets New Survival Benchmark for Advanced Biliary Tract Cancers

New data from the TOPAZ-1 study, published in the Journal of Hepatology, demonstrates sustained survival benefits for patients with advanced biliary tract cancers (BTCs) treated with durvalumab in combination with gemcitabine and cisplatin. This extended follow-up reinforces the regimen as the standard of care for first-line treatment of this challenging disease.
Understanding biliary Tract Cancers and the Need for Effective Treatments
Biliary tract cancers (BTCs) encompass a group of aggressive malignancies, including intrahepatic and extrahepatic cholangiocarcinoma, and also gallbladder cancer. A notable majority of individuals diagnosed with BTCs, up to 85%, present with advanced disease. At this stage, curative surgical options are often not feasible, and the prognosis is generally poor, with 5-year survival rates typically ranging from a mere 3% to 13%. Even in cases where surgery is considered perhaps curative, recurrence or death within five years remains a significant concern, affecting 57% of patients with extrahepatic BTC and a staggering 61% to 98% of those with intrahepatic BTC.These statistics highlight the critical unmet need for more effective therapeutic strategies.
TOPAZ-1 Study: Sustained Survival Benefits and Long-Term Follow-Up
The latest data from the TOPAZ-1 study represents the longest follow-up reported to date in advanced BTC research. This updated analysis, conducted approximately three years after the last participant enrolled, assessed overall survival and safety. The findings revealed that after three years, more than double the number of participants who received durvalumab plus gemcitabine and cisplatin were still alive compared to those treated with a placebo in combination with the same chemotherapy.
The authors emphasize that these results offer significant reassurance to both patients and practitioners regarding the long-term survival advantages of this treatment combination.
Investigator insights and Treatment Implications
Lead investigator Dr.Do-Youn Oh, from the Division of Medical Oncology at Seoul National University hospital, elaborated on the study’s objectives and findings.”Following on from the previous analysis, we assessed the length of time that participants survived after starting treatment as well as any serious side effects they experienced,” Dr. Oh stated. “We also examined the characteristics of participants still alive at least 30 months after starting treatment to see if any patterns were apparent in this group of extended long-term survivors. we assessed the subsequent anticancer therapy received by participants after finishing the study treatment and any impact on how long participants survived.”
Dr. Oh further commented on the impact of the TOPAZ-1 study, noting, ”The TOPAZ-1 study readouts had already changed the treatment landscape for advanced BTC. We are very pleased to report that the benefits of the durvalumab plus gemcitabine and cisplatin regimen persist after more than 3 years, and that the long-term survival benefits were observed without the need to continue chemotherapy over an extended period of time, as participants received a maximum of eight cycles of chemotherapy.”
Crucially, the analysis indicated that no specific subgroup was disproportionately driving or excluded from the extended long-term survivor population. This suggests a broad applicability of the treatment. “This implies that all patients with advanced BTC should be treated with durvalumab plus gemcitabine and cisplatin as participants appeared to have benefited from the treatment regardless of baseline characteristics,” Dr.Oh concluded. ”We hope the results will help physicians,patients,and caregivers make informed decisions for the treatment of advanced BTC.”
