The EHR Burden and the Promise of AI
Electronic Health Records (EHRs) have fundamentally changed clinical practice, streamlining data and standardizing care. Though, many physicians report feeling overwhelmed by the demands of EHRs – endless screens, constant alerts, and repetitive clicks that detract from patient interaction. As Becker’s Hospital Review reports, chief medical information officers (CMIOs) are hearing increasing concerns about clinician burnout related to EHR usability.
“Documentation is only the opening act,” said Mark Mabus, MD, CMIO of Parkview Health in Springfield, Missouri. “the real headliner is the nonstop alerts, the inbox pings, the endless clicks and the mental gymnastics that keep clinicians from being present with their patients.”
AI: From Add-On to Enterprise reality
CMIOs across the country are looking to artificial intelligence (AI) to alleviate this burden, with some anticipating a essential disruption of health IT. Neal Chawla, MD, CMIO at WakeMed in Raleigh, North Carolina, is preparing for AI’s transition from a helpful addition to an enterprise-wide necessity.
“It will be a while before AI approaches real maturity, even though it is already proving its benefit and growing within organizations,” dr. Chawla stated. “We all know about the risks with AI but have not seen it when AI is scaled enterprise-wide, which is where we are quickly headed.”
the Evolving Role of the CMIO
This shift is redefining the role of the CMIO. Previously focused on EHR optimization, CMIOs are now increasingly involved in AI governance – establishing guidelines, educating clinicians on responsible AI use (“trust but verify”), and implementing post-implementation monitoring.
“Our work will be building a practical, trustworthy framework for clinical AI while navigating the onslaught of everything AI,” said Bonnie Boles, MD, CMIO of Tanner Health in Carrollton, Georgia.
A Paradigm Shift: Software Adapting to Clinicians
Some CMIOs believe this represents a long-awaited change. K. Nadeem Ahmed, MD, CMIO of Valley Health System in Ridgewood,new Jersey,expressed frustration with years spent trying to force clinicians to adapt to complex software.
“All the energy CMIOs spend on trying to get clinicians to adapt to complex software systems will now be reversed: the software will adapt to the clinician,” Dr. Ahmed said. “This is the ultimate paradigm shift and I welcome it wholeheartedly.”
Beyond Generative AI: The Rise of Agentic Systems
The vision extends beyond generative AI models to “agentic AI” - systems capable of acting on behalf of both patients and clinicians.John-Paul Mead, MD, CMIO at Cayuga Health in Ithaca, New York, envisions AI agents handling initial patient calls, retrieving information, and improving the overall patient experience.
Genomics and Patient-Generated Data: New Frontiers
Even for organizations not solely focused on AI, scalability remains a key challenge. Eric Cheng,CMIO at UCLA Health in Los Angeles, identifies genomics as the next major hurdle, noting that while workflows for ordering, storing, and transmitting genomic data exist, they are not yet seamless.
Other leaders anticipate a shift towards patient-driven technology. C. Becket Mahnke, MD, CMIO of Confluence Health in Wenatchee, Washington, believes patients will increasingly adopt AI tools independently, allowing physicians to concentrate on more complex cases. Matthew Anderson, MD, CMIO at HonorHealth in Scottsdale, Arizona, predicts the proliferation of at-home diagnostics will necessitate new approaches to data validation and integration.
The Future of the CMIO: Governance and Integration
A common thread unites these perspectives: CMIOs are evolving beyond software optimization to strategically position their systems and clinicians for a future where technology is pervasive and actively participates in decision-making.
“With AI in every corner of the EHR, the CMIO’s role is shifting from EHR optimization to AI governance, ensuring safety, compliance, and true clinical value,” Dr. Boles said.
