Brain Scan Biomarker Forecasts Early-Onset Alzheimer’s Decline
Researchers at Massachusetts General Hospital and Harvard Medical School in Boston examined 130 patients aged 40 to 64 years. These participants had biomarker-supported sporadic early-onset Alzheimer disease at the mild cognitive impairment stage, alongside 97 cognitively normal controls. Led by Thiago Paranhos, M.D., the research team focused on the early-onset Alzheimer disease signature. This specific set of predominantly parietotemporal regions shows greater atrophy in these patients than in controls.
Hazard Ratios and Statistical Fit
Faster progression to dementia was predicted by greater baseline atrophy within the early-onset Alzheimer disease signature, yielding a hazard ratio of 1.24 per one-standard deviation increase in atrophy. Investigators also used the global Clinical Dementia Rating to measure clinical severity across participants.
When researchers added the early-onset Alzheimer disease signature atrophy burden to a model that already included baseline clinical severity, model fit improved significantly. This was demonstrated by a decrease in the Akaike Information Criterion of -4.5.
