A simple blood test may offer a window into the future for individuals at risk of developing Crohn’s disease, potentially detecting the condition years before the onset of debilitating symptoms. Researchers have identified a specific immune response that appears to precede the clinical manifestation of this chronic inflammatory bowel disease (IBD).

Crohn’s disease, affecting an estimated 1 million people in the United States, causes ongoing digestive issues, pain, and fatigue. Currently, diagnosis relies on identifying symptoms and utilizing imaging and endoscopic procedures to visualize inflammation in the gastrointestinal tract. However, these methods only detect the disease after it has already begun to cause damage. The new blood test aims to shift that paradigm towards proactive identification and, potentially, prevention.

The key lies in measuring the body’s immune response to flagellin, a protein found on gut bacteria. In a healthy digestive system, a diverse community of gut bacteria coexists peacefully. However, in individuals who develop Crohn’s disease, the immune system mistakenly attacks these normally harmless microbes. Previous research has shown elevated levels of antibodies to flagellin in patients already diagnosed with Crohn’s. The recent study, conducted by researchers at Sinai Health in Toronto, investigated whether these antibodies were also present in individuals at higher risk – first-degree relatives of Crohn’s patients – before they developed the disease.

The research team evaluated 381 first-degree relatives of individuals with Crohn’s disease. Over nearly two and a half years, 77 participants went on to develop the disease. Importantly, 28 of those who were ultimately diagnosed had elevated antibody responses to flagellin prior to symptom onset. The strongest responses were observed in siblings, suggesting a role for shared environmental factors in disease development.

“We wanted to know: Do people who are at risk, who are healthy now, have these antibodies against flagellin?” explained Dr. Ken Croitoru, a clinician scientist with Mount Sinai Hospital’s Center for Inflammatory Bowel Disease. “We looked, we measured, and yes at least some of them did.”

The findings, published in Clinical Gastroenterology and Hepatology, suggest that the immune reaction to flagellin may not be a consequence of Crohn’s disease, but rather a contributing factor in its development. This is a crucial distinction, as it opens the door to potential preventative strategies. If the antibody response precedes and contributes to the disease process, interventions aimed at modulating the immune system could potentially delay or even prevent the onset of Crohn’s.

The implications of this research extend beyond early diagnosis. Crohn’s disease incidence is rising, particularly among children, with an estimated 470,000 Canadians expected to be living with IBD by 2035, according to Crohn’s and Colitis Canada. A non-invasive blood test could significantly improve the management of this growing public health concern.

However, it’s important to note that the blood test is not a definitive predictor of Crohn’s disease. Not everyone with elevated flagellin antibodies will develop the condition. Further research is needed to refine the test’s accuracy and identify which individuals are most likely to benefit from preventative interventions. The test is also not yet widely available for clinical use.

Dr. Sun-Ho Lee, a clinician scientist and staff gastroenterologist at Mount Sinai Hospital’s Centre for Inflammatory Bowel Disease, emphasized the importance of understanding the interplay between gut bacteria and the immune system in the development of Crohn’s disease. This research represents a significant step forward in unraveling the complex mechanisms underlying this chronic condition.

While the prospect of preventing Crohn’s disease remains a future goal, this blood test offers a promising new tool for identifying individuals at risk and potentially intervening before irreversible damage occurs. The research underscores the importance of continued investment in understanding the intricate relationship between the gut microbiome and human health.
