Dentists may soon play a larger role in identifying individuals with undiagnosed diabetes and prediabetes, according to a new study published in the issue of the Journal of Dentistry. Researchers found that a simple finger-prick blood test administered during routine dental exams revealed elevated blood sugar levels in over one-third of patients, many of whom were unaware they were at risk.
The study, conducted by researchers at King’s College London in the U.K., involved over 900 patients undergoing dental examinations. Participants also received a hemoglobin A1C blood test, which measures average blood sugar levels over the preceding two to three months. Importantly, the test requires no fasting, making it convenient to administer during a dental visit.
The results showed that approximately 29% of patients had blood sugar levels indicative of prediabetes, a condition where blood sugar is higher than normal but not yet high enough to be diagnosed as diabetes. An additional 7% were found to have full-blown diabetes, and crucially, none of these individuals had previously been diagnosed with either condition.
“The findings suggest that dental visits may offer a valuable opportunity to identify those at risk of diabetes, particularly in older patients, those with higher BMI and people with gum disease,” explained Dr. Giuseppe Mainas, a periodontal specialist and research associate at King’s College London, in a news release. Body Mass Index (BMI) is a widely used measure of body fat based on height and weight.
The hemoglobin A1C test works by measuring the percentage of hemoglobin proteins that have glucose attached to them. The higher the blood sugar levels over time, the more glucose will be attached to the hemoglobin. A result of 6.5% or higher generally indicates diabetes, while levels between 5.7% and 6.4% suggest prediabetes.
Lead researcher Mark Ide, a professor of periodontology at King’s College London, emphasized the potential impact of this screening method. “When the test reveals high levels of (hemoglobin A1C), patients can then see their general practitioner to investigate further,” he said. “This represents something they might not have done without having the dental screening.”
The study highlights a potentially significant gap in diabetes detection. Many people with diabetes remain undiagnosed for years, leading to delayed treatment and increased risk of complications. These complications can include heart disease, kidney failure, nerve damage, and vision loss. Early detection is therefore crucial for managing the condition effectively and preventing long-term health problems.
The link between oral health and diabetes is well-established. People with diabetes are more susceptible to gum disease (periodontitis), and severe gum disease can, in turn, make it more difficult to control blood sugar levels. This creates a bidirectional relationship where each condition can worsen the other. The study suggests that dentists, by virtue of their regular patient contact and ability to perform a simple blood test, could become key players in breaking this cycle.
However, researchers acknowledge that chairside diabetes screening is not a replacement for traditional diagnostic methods. A positive screening result requires confirmation by a physician through further testing and evaluation. The role of the dentist is to identify potential cases and prompt patients to seek medical attention.
Further research is needed to determine the cost-effectiveness of implementing chairside A1C testing in dental practices and to assess patient acceptance of this type of screening. A survey of dental providers indicated that the amount of time required to obtain and discuss test results was a common barrier to offering the screening. Nevertheless, the study’s findings suggest that integrating diabetes screening into routine dental care could be a valuable public health strategy.
The findings come at a time when diabetes prevalence continues to rise globally. According to the Centers for Disease Control and Prevention (CDC), over 37 million Americans have diabetes, and approximately 96 million adults have prediabetes. Many of these individuals are unaware of their condition, underscoring the need for innovative approaches to early detection.
“Most of the patients in our study were surprised that they had elevated (hemoglobin A1C) levels and had no idea they might have prediabetes or diabetes,” Ide noted, highlighting the potential for dental screenings to reach individuals who might otherwise go undiagnosed.
