Adults who experienced strict sugar limits during early life, including in utero and through their first two years of development, face a substantially lower risk of chronic illnesses such as type 2 diabetes and hypertension, according to a study published on October 31, 2024, in the journal Science.

Researchers led by Dr. Tadeja Gracner at the University of Southern California utilized a unique historical natural experiment to examine the long-term health consequences of early-life nutrition. According to the National Institutes of Health, the team analyzed health outcomes for more than 60,000 individuals born in the United Kingdom between October 1951 and March 1956, a window spanning the end of wartime sugar rationing.

The Natural Experiment of Postwar Rationing

Great Britain maintained strict rationing on sugar and sweets during and after World War II, capping allowances at levels remarkably similar to current U.S. dietary guidelines. Adults during rationing were limited to less than 40 grams per day, children to less than 15 grams, and infants under two years received virtually no added sugar.

That environment changed abruptly in September 1953 when rationing officially ended, causing national sugar consumption to nearly double almost overnight. Individuals born in July 1954 or later were conceived after the restrictions lifted and never experienced rationing, providing researchers with a distinct control group. Participants born earlier experienced varying durations of sugar restriction, ranging from prenatal development up to their first two years of life.

Measurable Reductions in Chronic Disease Risk

When researchers evaluated UK Biobank data for participants who were aged 51 to 66 at the time of the survey, they found that early-life exposure to rationing yielded dramatic long-term protections. Risk for chronic conditions declined steadily with longer exposure, particularly for restrictions lasting longer than six months postnatally.

Furthermore, those protected individuals experienced a four-year average delay in the diagnosis of diabetes and a two-year average delay in the diagnosis of hypertension. Participants exposed solely in utero achieved roughly one-third of the risk reduction seen in those with the longest overall exposure.

Implications for Modern Dietary Guidelines

Current U.S. dietary guidelines advise against the consumption of added sugars from in utero up until age two. Despite these recommendations, national data show that most children encounter added sugar very early in life, sometimes transmitted before birth and through breastfeeding via the maternal diet.

Study authors noted that investigating the multi-decade impacts of added sugar has historically proved difficult due to the scarcity of randomized nutritional environments. It is hard to find situations where people are randomly exposed to different nutritional environments early in life, and follow them for 50 to 60 years. The end of rationing provided us with a novel natural experiment to overcome some of these challenges.

Additional commentary from Dr. Kriti Jain, a program official at the National Institute on Aging, emphasized the broader implications for prenatal and infant health. Future investigations will seek to pinpoint the precise biological mechanisms by which early-life sugar consumption translates into decades-later health consequences.
