A simple blood test could one day offer a crucial window into the earliest stages of Parkinson’s disease, potentially decades before the onset of noticeable motor symptoms like tremors. Researchers are reporting promising progress in identifying biological markers in the blood that signal the very beginnings of the disease process, offering hope for earlier diagnosis, and intervention.

Early Biological Changes Detectable in Blood

For years, Parkinson’s disease has been notoriously difficult to diagnose early. Diagnosis typically relies on identifying motor symptoms, by which point significant neurological damage has already occurred. However, new research suggests that changes related to DNA repair and cellular stress – hallmarks of Parkinson’s – leave detectable traces in the blood long before these symptoms manifest. A study led by Chalmers University of Technology in Sweden, in collaboration with Oslo University Hospital in Norway, has pinpointed these early biomarkers.

The research focuses on the processes of DNA repair and the cellular stress response. These processes can be disrupted for up to 20 years before the development of the characteristic motor symptoms of Parkinson’s, according to the study authors. By employing machine learning techniques, the researchers were able to identify patterns in blood samples linked to these processes, patterns that were not present in healthy individuals or those already diagnosed with the disease.

“We highlighted biomarkers that likely reflect some of the early biology of the disease and showed they can be measured in blood,” says Annikka Polster, a biostatistician at the University of Oslo. “This paves the way for broad screening tests via blood samples: a cost-effective, easily accessible method.”

The Promise of Early Detection and Intervention

The potential benefits of early detection are significant. Currently, treatments for Parkinson’s primarily focus on managing symptoms. However, identifying the disease in its earliest stages could open the door to interventions aimed at slowing or even preventing the progression of the disease. The researchers believe that understanding the mechanisms driving these early biological changes could also lead to the development of new therapies.

“We have found an important window of opportunity in which the disease can be detected before motor symptoms caused by nerve damage in the brain appear,” explains Annikka Polster. “The fact that these patterns only show at an early stage and are no longer activated when the disease has progressed further also makes it interesting to focus on the mechanisms to find future treatments.”

Understanding Parkinson’s Disease

Parkinson’s disease is a progressive neurological disorder that affects movement. While tremors are often the most recognizable symptom, the disease also commonly presents with slowed movement, muscle stiffness, balance problems, and changes in gait. As the disease progresses, it can impact thinking and memory. The underlying cause of Parkinson’s is the loss of dopamine-producing neurons in the brain.

Recent research suggests that problems with DNA maintenance and reduced cell resilience may contribute to this neuronal damage. The current study builds on this understanding by identifying specific biomarkers related to these cellular processes that can be detected in the blood.

Beyond Motor Symptoms: Recognizing Early Warning Signs

While motor symptoms are often the defining feature of Parkinson’s, some non-motor symptoms can appear years before. These can include a loss of smell, changes in handwriting (specifically, a shrinking of handwriting size, known as micrographia), and sleep disturbances, such as acting out dreams during REM sleep. It’s important to note that experiencing one or more of these symptoms does not necessarily mean someone has Parkinson’s, but it warrants a discussion with a healthcare professional.

Other early signs can include constipation, a stooped posture, and difficulty with balance. However, these symptoms can also be caused by a variety of other conditions, making early diagnosis challenging.

Looking Ahead

The research from Chalmers University of Technology and Oslo University Hospital represents a significant step forward in the quest for early Parkinson’s detection. While further research is needed to validate these findings and develop a reliable blood test for widespread clinical use, the potential impact on patient care is substantial. The ability to identify individuals at risk of developing Parkinson’s could revolutionize the way the disease is managed, offering the possibility of earlier interventions and improved outcomes.

The study involved tracking gene expression in blood samples from 188 healthy control participants, 393 individuals with fully developed Parkinson’s, and 58 people in the prodromal stage – the earliest phase of the disease. This comprehensive approach allowed researchers to pinpoint the specific biomarkers that distinguish those in the early stages of Parkinson’s from healthy individuals and those with more advanced disease.
