The Pathophysiology of Parkinson’s Disease
Parkinson’s disease is a chronic degenerative condition of the central nervous system that affects both motor and non-motor systems. The primary motor symptoms, collectively known as parkinsonism, are caused by the death of neurons in the substantia nigra. This area of the midbrain is responsible for supplying dopamine to the basal ganglia.
While the exact cause of this cell death is not fully understood, it involves the aggregation of alpha-synuclein protein into Lewy bodies within the neurons. The disease typically manifests in people over the age of 60, affecting approximately one percent of this population. When the disease occurs in individuals under the age of 50, it is classified as early-onset Parkinson’s disease.
The development of the disease is believed to be a combination of genetic and environmental factors. Individuals with an affected family member have a higher risk due to inherited genes. Environmental risk factors include previous head injuries and exposure to pesticides or trichloroethylene.
Symptom Progression and Diagnosis
Symptoms usually appear slowly, with motor issues often being the primary basis for diagnosis. Early motor symptoms include stiffness, tremors, slowness of movement, and difficulty walking. As the disease progresses, non-motor symptoms become more common, including issues with sleep, behavior, sensory systems, and thinking. Parkinson’s disease dementia is also a common and progressive aspect of the condition.

The average life expectancy following a diagnosis is between 7 and 15 years. Because there is no known cure, treatment focuses on relieving symptoms and maintaining function.
