Despite a relatively straightforward diagnostic test and effective treatment options, EoE is frequently missed or misdiagnosed, leaving sufferers to endure years of unnecessary discomfort.
Understanding eosinophilic Esophagitis
Eosinophilic esophagitis occurs when a higher-than-normal number of eosinophils – a type of white blood cell - accumulate in the esophagus. This inflammation damages the esophageal lining, leading to difficulty swallowing (dysphagia), food impaction (food getting stuck), chest pain, and even malnutrition.While the exact cause of EoE isn’t fully understood,it’s believed to be triggered by a combination of genetic predisposition and environmental factors,most commonly food allergies.
The Diagnostic Gap: Why is eoe Missed?
The delay in diagnosis stems from several factors. Symptoms can mimic other conditions, such as gastroesophageal reflux disease (GERD) or anxiety.Many healthcare providers aren’t fully aware of EoE or its presentation, leading to initial misdiagnosis and treatment focused on the wrong ailment. A 2023 study highlighted that awareness of EoE remains low, even among gastroenterologists.
Fortunately, diagnosing EoE is relatively simple. It begins with an upper endoscopy, a procedure where a thin, flexible tube with a camera is inserted into the esophagus to visualize the lining. during the endoscopy, biopsies are taken to examine under a microscope for the presence of elevated eosinophil levels. A threshold of 15 eosinophils per high-power field is generally used to diagnose EoE, though some experts suggest lower thresholds in certain cases.
Effective Treatments are Available
Once diagnosed, EoE is highly treatable. Treatment strategies are tailored to the individual and the identified triggers. The primary approaches include:
- dietary Therapy: Identifying and eliminating food allergens is a cornerstone of EoE management. This often involves an elimination diet followed by food challenges to pinpoint specific triggers. The most common allergens are milk, wheat, soy, eggs, peanuts, tree nuts, fish, and shellfish.
- Proton Pump Inhibitors (PPIs): These medications reduce stomach acid production, which can help to decrease esophageal inflammation.
- Topical Corticosteroids: Swallowed corticosteroids can directly reduce inflammation in the esophagus.
- Esophageal Dilatation: For patients with esophageal narrowing (strictures), this procedure can widen the esophagus to improve swallowing.
Recent advancements have also explored the use of biologic therapies, such as dupilumab, which targets specific immune pathways involved in EoE. Dupilumab received FDA approval in September 2022 for adults and in November 2023 for children aged 12 and older, offering a new treatment option for those who don’t respond adequately to other therapies.
Looking Ahead: Improving Awareness and Access to Care
As of September 2, 2025, the need for increased awareness of EoE among both healthcare professionals and the general public remains critical. Early diagnosis and appropriate treatment can substantially improve the quality of life for those affected.
