The administration released its Fiscal Year 2027 budget request on April 3, 2026, proposing significant reductions and a restructuring of U.S. Global health funding. The proposal impacts discretionary funding for programs managed by the State Department, the Centers for Disease Control and Prevention (CDC), and the National Institutes of Health (NIH).
Analysis of the request shows a substantial decrease in funding for the State Department’s Global Health Programs (GHP) account. The proposed budget for this account is $5.1 billion, which is $4.3 billion less than the Fiscal Year 2026 amount of $9.4 billion.
Restructuring of Disease-Specific Funding
The Fiscal Year 2027 request proposes a shift away from earmarked funding for specific illnesses. The administration intends to eliminate disease-specific accounts and provide the Department crucial agility to address the actual needs of each recipient country
across various infectious diseases, including Tuberculosis, Malaria, Polio, and HIV/AIDS.
According to the budget request, this restructuring is intended to strengthen global health security and protect Americans from disease by allowing the department to adapt to the specific needs of recipient countries.
Program Eliminations and Uncertainties
Certain health initiatives face complete removal in the new proposal. Funding for reproductive health and family planning (FP/RH) is specifically eliminated in the Fiscal Year 2027 request.
Other programs are not mentioned in the request, leaving their status uncertain. These include the vulnerable children program, nutrition programs, and funding for neglected tropical diseases (NTDs), meaning it is possible these activities are also eliminated.
Changes to Multilateral and Research Funding
The budget request alters the approach to multilateral funding, specifically regarding the Global Fund to Fight AIDS, Tuberculosis and Malaria. The request does not provide a specific funding amount for the Global Fund for Fiscal Year 2027.
Instead, the administration commits to leveraging $2 from other donors for every $1 from the United States
and stipulates that U.S. Funds may not exceed 33% of the total amount contributed to the Global Fund.
In addition to State Department cuts, the White House has proposed a $5 billion reduction for the National Institutes of Health (NIH) in the Fiscal Year 2027 budget request.
