Tokyo, Japan – A new study is shedding light on the evolving motivations of adults with diabetes as they engage with digital health interventions. Research conducted in Japan indicates that the initial enthusiasm generated by team-based gamification apps designed to promote self-care may peak around six months, before potentially declining, highlighting the need for adaptive intervention strategies to maintain long-term engagement.
The prospective, single-arm longitudinal study, registered as UMIN000044874, involved 29 adults diagnosed with diabetes – predominantly type 2 – recruited from outpatient clinics. Participants were asked to utilize a commercially available, team-based gamification application for at least seven days, designed to foster positive habits through peer support and social comparison. The primary focus of the research was to chart the trajectory of “autonomous motivation” – a key factor in sustaining self-care behaviors – over a 12-month period.
Autonomous motivation, rooted in self-determination theory, is considered crucial for long-term adherence to health regimens. The study utilized the Treatment Self-Regulation Questionnaire-Autonomous Motivation subscale (TSRQ-AM), with a scoring range of 7-49, to assess motivation levels at baseline, six weeks, six months, and one year. Researchers also tracked secondary measures including hemoglobin A1c (HbA1c), body weight, triglyceride levels, and various psychological scales assessing self-efficacy, diabetes self-care activities, problem areas related to diabetes, and overall well-being.
Initial findings revealed a significant increase in mean TSRQ-AM scores from 37.4 (SD 7.9) at the study’s outset to 39.5 (SD 7.4) at the six-month mark, representing a moderate effect size (Cohen d=0.47). Interestingly, the analysis indicated a non-linear pattern of change. A statistical model incorporating a quadratic time term demonstrated a significantly better fit than a linear-only model (χ21=4.1; P=.04), suggesting an inverted U-shaped trajectory. This implies that autonomous motivation may peak around six months into using the app, after which it could begin to decline.
Alongside motivational shifts, the study observed a statistically significant decrease in body weight over the year (b=-0.39; P=.01). However, no significant changes were detected in HbA1c (P=.40) or triglyceride levels (P=.14). Researchers also found that individuals with a higher baseline Body Mass Index (BMI) tended to exhibit lower TSRQ-AM scores (b=-1.00; P=.001), suggesting that weight may be a factor influencing initial motivation levels.
The findings, published in Formative, underscore the complexities of sustaining motivation in chronic disease management. While gamification and mobile health interventions hold promise for enhancing engagement, the study suggests that a “one-size-fits-all” approach may not be sufficient. The observed curvilinear pattern highlights the importance of designing interventions that can adapt to changing motivational states over time.
“This formative study provides preliminary evidence of a nonlinear, 1-year trajectory of autonomous motivation following the introduction of a team-based app,” the researchers concluded. “The observed curvilinear pattern suggests that autonomous motivation during the intervention may peak at around 6 months, underscoring the importance of adaptive intervention designs to maintain engagement over time.”
The study’s authors acknowledge that the single-arm design limits the ability to draw definitive conclusions about causality. Further research, including randomized controlled trials, is needed to confirm these findings and to explore the optimal strategies for maintaining long-term motivation in individuals with diabetes. The accompanying reduction in body weight, they note, warrants further investigation to determine its physiological relevance and potential connection to the observed motivational patterns.
The increasing prevalence of diabetes globally – currently affecting an estimated 537 million adults worldwide, according to the International Diabetes Federation – underscores the urgency of identifying effective strategies for promoting self-management. Digital health interventions, including gamification, are increasingly being explored as potential solutions, but this research suggests that careful attention to the temporal dynamics of motivation is essential for maximizing their impact.
The study’s focus on a Japanese population also raises questions about the generalizability of the findings to other cultural contexts. Further research is needed to determine whether similar motivational trajectories are observed in diverse populations with varying cultural norms and healthcare systems.
