Changes to variable pay spark debate among medical staff.
A director at the IRCCS Casa Relief of the Suffering recently clarified the revised distribution criteria for variable compensation, a system that had been in place for two decades, where payments were reportedly made to all doctors regardless of performance. The manager, appointed in November 2022 to address the Foundation’s financial situation, presented the proposal to the Puglia regional Council’s Commission III ‘Healthcare Assistance Social Services’.
The manager emphasized that the anticipated shortfall from the ‘south decontribution,’ impacting the budget by €7.8 million, necessitates further cost reductions. In October-November 2024, we had to roll up our sleeves. We are making a further effort for the reduction of costs, also relating to human resources and trade union agreements that hypothesize reductions or different ways of the donation of the variable remuneration part. We have already reached the agreement with the staff of the sector (about 2,200 workers) and the agreement with the non-medical staff (about 200 workers),
the manager confirmed.
The measure is expected to affect the IRCCS budget by €1.4 million annually. The manager stated, The question we asked ourselves was: is it possible that a variable component has been paid 100% and 100% of people, totally and regardless of the results obtained by the Foundation? In my opinion and in our opinion, no.
The general manager explained,If the Foundation obtains economic results that contribute to balancing the budget, without profit concerns or resource distribution issues, the variable portion can be distributed. If those results are not achieved, the variable portion should not be distributed. I understand that this may displease those who have received it for 20 years.
Approximately 470 to 480 doctors could see a reduction of €230 gross per month, equivalent to €120 net. Tho, the manager announced that in 2025, €4.9 million of the real variable part, linked to the objectives to be achieved, has been made available.
Italian Healthcare Compensation: Understanding the Recent Changes
What’s New with Doctor Compensation in Italian Healthcare?
Changes to variable pay have sparked debate among medical staff, specifically at the IRCCS Casa Relief of the Suffering.
Why Are These Changes Happening?
The primary driver for these changes is financial. The IRCCS is facing notable financial challenges, including an anticipated shortfall from the ‘south decontribution,’ impacting its budget by €7.8 million. This, in turn, necessitates cost reductions.
How Does the Revised Compensation Structure Work?
The new compensation structure at the IRCCS Casa Relief of the Suffering, which was in place for two decades, changes how variable compensation for doctors is distributed. Under the previous system, payments were reportedly made to all doctors regardless of their individual performance. The revised structure ties variable pay to the economic results obtained by the Foundation. If economic results contribute to balancing the budget, without profit concerns or resource distribution issues, the variable portion can be distributed. If these results are not achieved, the variable portion will not be distributed.
Who Is Affected by These Changes?
This new measure affects approximately 470 to 480 doctors.
What are the Potential Financial Impacts?
the measure is expected to affect the IRCCS budget by €1.4 million annually. Doctors could see a reduction of perhaps €230 gross per month, equivalent to €120 net.Though, in 2025, €4.9 million of the real variable part, linked to the objectives to be achieved is now available.
