Heart Failure Accelerates Cognitive Decline, Study finds

Heart failure, affecting over six million Americans, may substantially increase the risk of early cognitive decline, research indicates. A Michigan Medicine-led team tracked nearly 30,000 adults, comparing cognitive function in those with and without heart failure.
The study, published in Circulation: Heart Failure, revealed a notable cognitive decrease at the time of heart failure diagnosis. furthermore, global cognition and executive functioning declined more rapidly post-diagnosis. Patients experienced a cognitive decline equivalent to 10 years of aging within seven years of diagnosis.
According to the research, typical risk factors for cognitive impairment, including high blood pressure and heart attack, did not fully account for the accelerated decline observed in heart failure patients. The most notable cognitive decline occurred among older adults, women, and white participants.
Researchers suggest that adults with heart failure may reach the threshold for meaningful cognitive decline almost six years earlier than those without the condition. Executive functioning could diminish about four-and-a-half years sooner.
“Heart failure is a disease that never goes away and treating it relies heavily on a patient’s ability to follow specific instructions,monitor their symptoms and keep up with many different medications,” said Supriya Shore,MBBS,MSCS,of the University of Michigan Medical School.
Shore emphasized the importance of early cognitive assessment, stating it “should be a warning for providers to assess a patient’s cognitive ability early and factor it into the care plan.”
“Regular cognitive monitoring of older adults with heart failure would help identify individuals with the earliest signs of cognitive decline who require supportive care,” said Deborah A. Levine, M.D., M.P.H., also of U-M Medical School.
Levine added that further research is needed to understand the mechanisms driving this decline and to develop effective interventions.
Shore noted that treatment decisions are complex and heavily reliant on a patient’s cognitive capacity. The findings also shed light on a previous study indicating that many heart failure patients have a poor understanding of their prognosis,while caregivers often recognize health declines.
“Most people with heart failure, as well as their care partners, want discussions around prognosis to begin early in the course of the disease and to be repeated routinely using patient-centered language,” Shore said.
