Better cardiovascular health, as measured by American Heart Association metrics, is linked to a lower risk of mortality in postmenopausal women, according to an association analysis study published in Menopause. The research highlights the increasing risk for cardiovascular disease and death after menopause due to hormonal changes, metabolic alterations, and vascular aging.
The study focused on Life’s Essential 8 (LE8) and Life’s Crucial 9 (LC9) scores, industry-accepted measures of overall cardiovascular health. LC9 expands on LE8 by incorporating measures of psychosocial health.
Cardiovascular Health Scores and Mortality Risk
Researchers found that higher scores on both LE8 and LC9 were associated with a reduced risk for both all-cause and cardiovascular mortality among postmenopausal women. This association was partially explained by factors related to adiposity and systemic inflammation, suggesting these play a role in the link between heart health and longevity.
Cardiovascular disease is the leading cause of morbidity and mortality in women globally. The period after menopause brings significant physiological changes that elevate this risk, making comprehensive cardiovascular health assessments crucial for identifying high-risk individuals and developing targeted prevention strategies.
Understanding Life’s Essential 8 and Life’s Crucial 9
Life’s Essential 8, developed by the American Heart Association, assesses cardiovascular health based on eight factors: diet, exercise, nicotine exposure, sleep, weight, cholesterol, blood sugar, and blood pressure. Life’s Crucial 9 builds upon this framework by adding psychosocial health, encompassing factors like social support and mental wellbeing.
While previous research has shown an association between higher LE8 and LC9 scores and improved cardiovascular outcomes, the underlying mechanisms driving these associations have not been fully understood. This new study sheds light on the potential mediating roles of adiposity and inflammation.
Adiposity, Inflammation, and Cardiovascular Risk
Postmenopausal women often experience shifts in body composition and fat redistribution, particularly around the midsection, which can increase cardiometabolic risk. This increased adiposity is frequently linked to higher levels of inflammatory markers and metabolic dysregulation.

The study suggests that these changes in adiposity and inflammation partially explain why better cardiovascular health scores are associated with lower mortality rates. Addressing these factors may be key to improving cardiovascular health and extending lifespan in postmenopausal women.
Study Details and Publication
The findings were published online today, April 29, 2026, in Menopause, the journal of The Menopause Society. The association analysis study examined data from a cohort of postmenopausal women to investigate the relationship between LE8 and LC9 scores, mortality risk, and potential mediating factors.
Further research is needed to fully elucidate the complex interplay between cardiovascular health, adiposity, inflammation, and mortality in postmenopausal women. However, this study provides valuable insights into the importance of comprehensive cardiovascular health assessment and targeted interventions to reduce risk.
