Health experts are warning about a dangerous “clinical blind spot” in heart disease detection among young women, emphasizing that atypical symptoms often lead to delayed diagnosis and increased risk of complications.

Cardiovascular disease remains the leading cause of death for women in the United States and globally, yet many young women do not recognize their risk. A 2019 study cited by the Cleveland Clinic found that only 50% of women under age 55 who experienced a heart attack believed they were at risk beforehand, despite having multiple risk factors.

Women frequently experience heart attack symptoms that differ from the classic chest pain commonly associated with men. These atypical symptoms can include shortness of breath, nausea, vomiting, back or jaw pain, and unexplained fatigue, which may be mistaken for less serious conditions such as indigestion, stress, or menopause.

One particularly concerning condition is Spontaneous Coronary Artery Dissection (SCAD), which occurs when a tear forms in the wall of a coronary artery. SCAD is more common in women who appear healthy and lack traditional risk factors such as high cholesterol or smoking history, making it difficult to detect until significant damage has occurred.

Mental health factors such as depression and anxiety are also increasingly recognized as significant contributors to heart disease risk in younger women, further complicating diagnosis when physical symptoms are vague or absent.

Healthcare providers may fail to consider heart disease as a possibility in young women presenting with non-specific symptoms, leading to underdiagnosis or misattribution of symptoms to anxiety or other non-cardiac causes. This delay in recognition increases the likelihood of complications and long-term damage.

Experts stress that awareness of subtle symptoms and regular heart check-ups are critical for early detection. Preventive measures include maintaining a heart-healthy lifestyle, managing blood pressure and cholesterol, avoiding smoking, and discussing personal and family medical history with a healthcare provider.

While public health campaigns have improved awareness of heart disease in women, gaps remain in recognizing how symptoms present differently across sex and age groups. Continued education for both patients and clinicians is essential to close the clinical blind spot and improve outcomes for young women at risk.
