Organ Transplant Shortage and HIV-Positive Donors

Art Caplan, from NYU Grossman School of Medicine, discusses the significant shortage of organs for transplantation. The need for organs like livers, hearts, kidneys, and lungs greatly exceeds supply, particularly for cadaver organs. More people are eligible for transplants now, including sicker and older individuals, which increases the demand.

One proposed solution to address this shortage is to use organs from HIV-positive donors. This approach involves giving these organs to recipients who do not have HIV but may be at risk of dying without a transplant. Recipients must understand the risks and be willing to accept this option.

The National Institutes of Health (NIH) has conducted studies on organ transplantation from HIV-positive donors to HIV-negative recipients. To reduce the risk of HIV transmission, recipients can take antiretroviral therapy (ART) and preexposure prophylaxis (PrEP). These treatments can keep HIV undetectable in most people and help prevent infection.

Before proceeding with such transplants, it’s crucial to ensure informed consent. Patients must understand the potential for HIV transmission and its treatment. They also need to be aware of lifestyle changes, such as avoiding intimate contact with others.

Despite the short-term success of these treatments, the long-term effects of HIV-positive organ transplants remain unknown. There has not been a long enough study period to fully understand the implications. Open communication about these risks is essential.

For many patients facing death due to organ shortages, the option of an HIV-positive donor could provide hope. It may help create a larger pool of donors, helping more people live longer, healthier lives.
