Portsmouth, NH – A coroner has issued a stark warning after a woman died from complications following surgery, highlighting systemic failures in patient care at Portsmouth Hospitals University NHS Trust. 62-year-old Susan Evans underwent a surgical procedure at the hospital. While initially discharged, she was readmitted weeks later suffering from severe abdominal sepsis.

Coroner Ms. Olsen, presiding over the inquest into Ms. Evans’ death, stated that Ms.Evans was still experiencing pain upon her initial discharge. “she was readmitted to the hospital on July 15, 2023. By this point, she was extremely unwell with abdominal sepsis from an anastomotic leak,” Ms. Olsen explained.

Despite receiving medical attention upon readmission, Ms. Evans’ condition tragically deteriorated, ultimately leading to her death. The coroner believes that had proper procedures been followed after the initial surgery,Ms. Evans would have likely remained hospitalized and received timely treatment.

“There is a risk that future deaths could occur unless action is taken,” Ms. Olsen warned, emphasizing the urgency of addressing the identified shortcomings.

Dr. John Knighton, chief Medical Officer at Portsmouth Hospitals University NHS Trust, expressed remorse for the care provided to Ms. Evans and extended condolences to her family. “I am very sorry for the care we provided to Susan and would like to send my deepest condolences to her family,” Dr. Knighton said.

He assured the public that the hospital is fully cooperating with the coroner’s examination and is committed to implementing changes to prevent similar tragedies. “A full internal investigation has also taken place,and we are taking action to improve our care,especially the communications between clinical teams and our patients when they leave the hospital,” Dr. Knighton stated.
