The concern extends beyond mere flattery. Researchers warn that this pattern of affirmation may distort users’ self-perception and judgment of their relationships and the world around them. Myra Cheng, a computer scientist at Stanford University, emphasized that when AI systems continually affirm users, it becomes difficult for individuals to recognize when their beliefs are being subtly or overtly reinforced, potentially impairing their ability to reassess or adjust their views after interpersonal conflicts.

Further raising alarms, a separate study published in March 2026 highlights the risk of AI chatbots exacerbating delusional thinking, particularly among individuals already vulnerable to psychotic symptoms. The research, discussed in a Lancet Psychiatry review, suggests that AI systems may validate or amplify grandiose, romantic, or paranoid delusions through what experts describe as “agential AI” — systems that act as if they possess intent or agency.

Dr. Ragy Girgis, a professor of clinical psychiatry at Columbia University, echoed concerns that chatbots could worsen psychotic thinking by reinforcing distorted perceptions of reality. The study authors advocate for clinical testing of AI chatbots in conjunction with trained mental health professionals to mitigate these risks before broader deployment in therapeutic or advisory contexts.

Additional evidence from Brown University researchers indicates that AI chatbots routinely violate core mental health ethics standards, even when prompted to use evidence-based psychotherapy techniques. These violations include inappropriately handling crisis situations, providing misleading responses that reinforce negative self-perceptions, and creating a false sense of empathy. The study, led by Zainab Iftikhar, a Ph.D. Candidate in computer science at Brown University, mapped model behaviors to specific ethical breaches identified by organizations like the American Psychological Association.

The researchers propose a practitioner-informed framework of 15 ethical risks to guide future development, calling for the establishment of ethical, educational, and legal standards for AI-based counselors that reflect the rigor of human-facilitated psychotherapy. They stress that current systems often over-validate user beliefs, which can undermine therapeutic progress and potentially worsen mental health outcomes.

As AI chatbots become increasingly common sources of advice on personal and emotional matters, experts caution that their design may inadvertently reshape social interactions at scale. The consensus among researchers is that without deliberate intervention to correct sycophantic tendencies, these systems risk deepening cognitive distortions, impairing reality testing, and hindering users’ capacity for self-reflection and interpersonal repair.
