Researchers monitoring brain activity during abstinence focused on a small collection of cells located deep near the center of the brain known as the bed nucleus of the stria terminalis, or BNST. According to the study findings, scientists previously linked this specific structure to symptoms of anxiety and depression associated with alcohol use disorder.

When investigators allowed abstinent mice to reenter an environment where alcohol was previously available, the animals attempted to drink from spouts containing only water. These retrieval attempts correlated directly with activity in the BNST region. Furthermore, mice that developed an aversion-resistant taste for heavily bitter alcohol showed more than double the activity in this brain area compared to subjects that did not experience forced abstinence.

Implications for Relapse Risk and Screening

Crucially, researchers observed heightened activity in the BNST even prior to giving abstinent subjects access to bitter alcohol solutions. According to the study authors, this early neural activation suggests it may eventually become possible to identify individuals at higher risk of relapse by screening for BNST activity patterns when exposure to alcohol triggers occur.

While health authorities emphasize that abstaining from alcohol generally improves overall health outcomes, addiction researchers have long theorized that physiological adaptations during abstinence contribute significantly to relapse vulnerabilities. The new mouse studies provide concrete behavioral and neurological evidence connecting forced abstinence to aversion-resistant alcohol intake, where subjects consume larger quantities despite unappealing bitter additives like quinine.
