The mimic: Hypokalemia and Stroke-Like Symptoms
A 61-year-old man presented to the emergency department exhibiting symptoms strikingly similar to a stroke: weakness on one side of his body and difficulty speaking. Initial assessments, including a CT angiogram, suggested a blockage in a major artery supplying the brain – a large vessel occlusion (LVO). This finding promptly raised concerns about the need for urgent intervention to restore blood flow.
However, further investigation revealed a surprising culprit: severe hypokalemia, or critically low potassium levels. His potassium level was measured at 1.9 mEq/L, significantly below the normal range of 3.5 to 5.0 mEq/L. This extreme deficiency was causing neurological symptoms that closely resembled a stroke.
Why Potassium Matters for Neurological Function
Potassium is an essential electrolyte vital for proper nerve and muscle function.
