The Anatomical Divide in Hypospadias Repair

Distal cases, where the urinary opening sits near the tip of the penis, are generally more straightforward to repair. Proximal cases feature openings along the shaft or at the scrotum, demanding intricate techniques to achieve normal anatomical function and appearance.

The Innovation of the TIP Technique

Early in his career practicing general urology with his father, Dr. Warren Snodgrass observed that traditional operations taught during his training resulted in numerous complications and an abnormal appearance.

To address this, he developed the Tubularized Incised Plate, or TIP, repair for distal hypospadias, publishing his findings in 1994. The technique quickly gained international adoption and became the most common operation worldwide for distal hypospadias, leading surgeons to rename it the Snodgrass repair.

Shifting Focus to Proximal Cases

Following his initial success with distal cases, Dr. Snodgrass sought to apply the TIP technique to proximal hypospadias. Limited patient volume in West Texas prompted a move to Seattle Children’s Hospital for a pediatric urology fellowship.

He later relocated to Dallas to study severe proximal hypospadias. He recognized that TIP only worked effectively when the penis was straight. Because most boys with proximal hypospadias also suffer from a bent penis known as chordee, and popular methods like the Byars flap caused frequent complications, Dr. Snodgrass and Dr. Bush shifted toward grafting the urethra.

Data Collection and the STAG Repair

The two surgeons refined the grafting approach into what they named the STAG repair, which stands for Straighten And Graft.

To track their outcomes meticulously, Dr. Snodgrass established a comprehensive patient database upon moving to Dallas, while Dr. Bush earned a master’s degree to better manage quality data collection and clinical review. Today, this database ranks among the largest in the world for hypospadias repair, allowing the team to continuously evaluate their results and refine their methods.

Advanced Protocols and Dedicated Practice

Analysis of their extensive database revealed healing challenges in certain grafts.

Additionally, for severe cases or patients with previous unsuccessful surgeries, they established a staged approach—separating the straightening procedure from the grafting procedure—which their anesthesiologist dubbed the Snodbush repair. Operating out of the Hypospadias Specialty Center, Dr. Snodgrass and Dr. Bush now dedicate their practice entirely to hypospadias repairs across all ages, aiming to correct abnormal anatomy into a normal penis on the first attempt and provide solutions for failed prior operations.
