Improving Access and Treatment Rates for Heart Failure Patients

Medical professionals are working to close the gap between clinical guidelines and actual practice in the treatment of heart failure (HF), specifically regarding the implementation of quadruple therapy. While clinical tools for diagnosis and treatment have advanced, mortality and morbidity rates for heart failure have remained steady, and the rates of patients receiving optimal medical therapy remain low.

The current standard of care for heart failure involves quadruple therapy, a combination of four medications designed to significantly reduce mortality and improve the quality of life for patients. Despite the established benefits of this regimen, fewer than 10% of patients receive it.

The Challenge of Guideline Adherence

A review published in the Korean Circulation Journal in March 2023 highlights that a persistent gap exists between clinical practice and established guidelines. This discrepancy underscores the necessity of quality assessments in heart failure care to improve patient outcomes.

The American Heart Association (AHA) has addressed these challenges through the Get With The Guidelines® – Heart Failure program. This initiative provides hospitals with registry tools and benchmarking assets to help clinical teams stay aligned with evolving guidelines.

Nearly 600 hospitals in the United States utilize this program to support adherence to treatment standards. Some participating facilities have reported significant improvements in patient outcomes; for example, one clinical nurse specialist noted that their facility reduced readmissions from 25% to 16%.

Implementing Quadruple Therapy

Quadruple therapy is now recognized as the new standard for heart failure treatment. Dr. Lee Goldberg, MD, MPH, describes these four medications not as a burden to the patient, but as a gift.

The 2022 AHA/ACC/HFSA Guideline for the Management of Heart Failure provides the contemporary evidence-based recommendations used to treat these patients. However, the transition from guideline publication to bedside implementation often takes time, which is why programs like Get With The Guidelines aim to accelerate the process.

Efforts to improve access to Guideline-Directed Medical Therapy (GDMT) are also focusing on specific patient populations. Recent discussions from the American College of Cardiology suggest that addressing modifiable barriers may improve access to quadruple GDMT for veterans with heart failure with reduced ejection fraction (HFrEF).

Strategies for Reducing Readmissions

Beyond medication adherence, healthcare providers are exploring various methods to prevent patient readmission. One such strategy involves the use of text reminders, which may help heart failure patients avoid returning to the hospital.

The integration of these tools—ranging from professional registry assets to patient-facing reminders—is intended to create a more comprehensive management system. By leveraging these tools, hospitals can better assess their programs and understand how their internal processes measure up against national standards.

The goal of these quality improvement strategies is to ensure that the benefits of contemporary medical research, such as the mortality reduction associated with quadruple therapy, reach the broadest possible population of patients.
