Research into schizophrenia spectrum disorders (SSDs) is increasingly focusing on the role of immune-inflammatory activation. Recent studies have explored the use of complete blood count (CBC)-based inflammation indexes as cost-effective and reproducible markers to assess the inflammatory status and symptom severity of patients.
One such study, which included inpatients with SSDs aged 18-65 hospitalized between May 2020 and March 2024, investigated the relationship between specific inflammatory ratios and the severity of clinical symptoms. Findings on Monocyte-to-Lymphocyte Ratio

The analysis revealed a significant relationship between the monocyte-to-lymphocyte ratio (MLR) and the severity of both positive and negative symptoms.
Specifically, the MLR showed a coefficient of 0.19 (p=0.048) regarding positive symptoms and a coefficient of 0.27 (p=0.004) regarding negative symptoms. In contrast, the study found no significant relationship between symptom severity and either the neutrophil-to-lymphocyte ratio (NLR) or the platelet-to-lymphocyte ratio (PLR).
The findings suggest that the MLR may serve as a more precise indicator of symptom severity in individuals with SSDs than other CBC-based ratios.
