Innovive Health, a behavioral home health firm, is utilizing a data-driven, patient-centered model to provide skilled nursing services to vulnerable and underserved populations across Massachusetts, Colorado, and Iowa.

The company’s operational approach focuses on complex mental health patients, employing a model designed to transform healthcare through quality outcomes delivered directly in home settings.

Emergency Response and Field Operations

The practical application of Innovive Health’s commitment to patient independence was demonstrated during a blizzard that struck the Northeast at the end of February 2026.

To maintain continuity of care during the storm, nurses Caroline Manzone and William Doherty used a snowmobile to reach patients in Fall River, Massachusetts.

The urgency of these efforts is tied to the specific needs of the patient population, many of whom manage multiple medical comorbidities alongside complicating behavioral illnesses.

Michelle Haskins, the vice president of clinical services at Innovive Health, noted that nurses are often the only individuals these patients see, making their presence critical during extreme weather to ensure patients have food, electricity, and safety instructions.

Community-Based Care Implementation

Beyond the use of specialized transport, the firm’s staff utilized foot travel to ensure patient safety in urban environments.

Nurse Jeanne ChaCha traveled on foot for 30 minutes from her home in the Charlestown neighborhood of Boston to visit a blind patient in Cambridge, Massachusetts.

This intervention was necessary because the patient’s aides were unable to reach him due to the blizzard conditions.

Strategic Impact on Healthcare Delivery

The objective of Innovive Health’s model is to enable patients to remain as independent as possible within their communities and reduce the frequency of hospitalizations.

According to Haskins, the services provided allow patients to be successful at home and integrated into the community, which directly impacts their overall quality of life.

If a lot of our patients didn’t have our services, I think that they would not be able to be successful at home or they would be in the hospital way more than they are

Michelle Haskins, vice president of clinical services at Innovive Health

By combining highly-trained staff with a comprehensive care model, the organization addresses individual patient needs to facilitate recovery and health outside of traditional clinical settings.
