Italian prosecutors have opened a double murder investigation into the deaths of a 50-year-old woman and her 15-year-old daughter after toxicology reports revealed the presence of ricin in their systems.
Antonella Di Ielsi and her daughter, Sara Di Vita, fell ill following a meal in their hometown of Pietracatella, a municipality located approximately 260 kilometers south-east of Rome. The pair were treated at Cardarelli Hospital in southern Italy on December 24, 2025, but died two days later.
Investigation and Findings
The case was initially treated as a medical emergency involving suspected food poisoning. Hospital staff believed the illness may have originated from seafood or mushrooms traditionally served during the holiday period. Because of this initial assessment, the mother and daughter were discharged from the emergency room on December 24.
Following their discharge, the symptoms of both victims worsened rapidly, leading to their readmission to the hospital. Dr. Vincenzo Cuzzone, head of the intensive care unit at Cardarelli Hospital in Campobasso, stated that the patients suffered liver failure followed by multi-organ failure at truly unparalleled speed

The nature of the investigation shifted after laboratory tests conducted in Italy and Switzerland identified ricin in the blood of Di Ielsi and Di Vita. Ricin is a highly toxic chemical naturally found in castor beans. the ingestion of a small amount can lead to rapid organ failure and death within three days.
Legal Proceedings and Suspects
Prior to the discovery of the poison, five doctors at Cardarelli Hospital were placed under investigation for manslaughter. They were accused of medical negligence for discharging the victims from the emergency room while they were ill.

With the confirmation of ricin poisoning, prosecutors have now transitioned the case into a double murder probe. While the investigation is active, authorities have stated they do not yet have a suspect.
Prosecutor Elvira Antonelli, who is leading the investigation, reported that blood analysis was also conducted on other family members. The victims’ father and husband, Gianni—the former mayor of Pietracatella—had been taken to the hospital but recovered. Analysis of his blood, as well as that of the couple’s 19-year-old daughter who was not present during the meal, showed no traces of ricin.
Medical Context of Ricin Poisoning
According to the US Centers for Disease Control and Prevention (CDC), the symptoms of ricin poisoning vary based on whether the substance was inhaled, injected, or ingested. When eaten, ricin can cause:
- Vomiting and diarrhea
- Seizures
- Bleeding in the gut
- Severe organ damage
The CDC further notes that there is currently no known cure or antidote for ricin poisoning, although supportive medical care can be administered to lessen the effects of the toxin.
