The infant had received four vaccines at Posyandu Melati in Bangkinang Kota on August 20, 2026: the second dose of DPT-Hb-Hib, the first dose of PCV, the third dose of OPV, and the second dose of Rotavirus, according to detikHealth records. Approximately seven hours after the administration, the baby developed a fever accompanied by shortness of breath and repeated seizures.

Family members brought the child to RSUD Bangkinang for emergency medical care.
Health Offices Investigate Protocols as National Immunization Continues
Following the serious adverse event report, the Riau Provincial Health Office, Komda KIPI Riau, and the Kampar Regency Health Office initiated an on-the-ground inquiry. Investigators examined patient records, immunization procedures, vaccines, and cold chain storage integrity to assess protocol adherence.

Ministry of Health Immunization Director Indri Yogyaswari emphasized that the national immunization schedule remains fully operational across the country. Indri noted that vaccines utilized in public programs undergo strict evaluation and supervision by the Indonesian Food and Drug Authority (BPOM), supported by ongoing surveillance through the KIPI reporting system.
