Left or Right? Experts Reveal the Best Side to Sleep On

Choosing which side to sleep on may significantly influence nighttime digestion, respiratory health, and acid reflux symptoms, according to medical research. While individuals spend roughly a third of their lives in bed, the chosen sleeping position remains largely instinctive rather than calculated. However, medical literature highlights distinct physiological differences between sleeping on the left side versus the right side.

Anatomical Differences and Digestive Impact

The human stomach sits slightly on the left side of the abdomen rather than directly in the center. According to Cédric Ben Chemhoun, a dietitian and nutritionist specializing in the intestinal microbiota interviewed by Santé Magazine, available literature consistently points to the left side as the optimal choice for digestion. When a person lies on their left side, gravity helps facilitate the movement of gastric contents from the stomach into the small intestine along the natural anatomical pathway.

Ben Chemhoun notes that while the stomach regulates its own emptying independently of gravity, lateral positioning provides a helpful supportive mechanism. Conversely, sleeping on the right side can increase the likelihood of acid reflux. According to findings cited by the Mayo Clinic and The Journal of Clinical Gastroenterology, right-side sleeping allows gastric acid to make more contact with the esophagus.

Scientific Findings on Gastroesophageal Reflux

Gastroesophageal reflux disease affects approximately 30 percent of the French population occasionally and 10 percent regularly, causing nighttime symptoms that disrupt sleep quality. Scientific studies confirm that left lateral decubitus positioning significantly improves nocturnal acid reflux symptoms. A systematic review and meta-analysis published in PubMed in October 2023 demonstrated that sleeping on the left side associates with shorter esophageal acid exposure times, faster acid clearance, and fewer overall reflux episodes compared to sleeping on the back or the right side.

These findings build on earlier research. A 1999 study published in the American Journal of Gastroenterology concluded that left lateral positioning remains preferable for patients suffering from nocturnal reflux, noting that right-side positioning correlates with higher acidity levels and slower esophageal acid clearance. When individuals rest on their left side, the stomach and pancreas align in a way that minimizes pressure on the lower esophageal sphincter, the muscular valve separating the stomach from the esophagus.

Circulation, Respiration, and Overall Comfort

Beyond digestion, lateral sleeping positions impact other bodily systems. Research highlights that sleeping on the left side can enhance blood circulation, a benefit particularly relevant for pregnant women. The National Health Service indicates that left-side sleeping improves venous return and reduces pressure on certain organs.

Regarding respiratory health, the American Academy of Sleep Medicine reports that sleeping on either side helps reduce snoring and limits episodes of sleep apnea compared to sleeping flat on the back, which often causes airway obstruction. Meanwhile, right-side sleeping remains an option for individuals who find it more comfortable for muscular or joint reasons, or for certain cardiac conditions, despite its drawbacks for acid reflux. Sleep specialists emphasize that alignment of the spine, a supportive pillow, and a quality mattress ultimately dictate healthy sleep architecture, meaning individuals troubled by digestive or respiratory symptoms may benefit from testing the left side without forcing an unnatural posture.
