Emcara Health, formerly PopHealthCare, collaborates with health⁤ plans to deliver primary and supportive care, including annual wellness checks, within patients’ homes. Their services encompass chronic condition management,lab work,and advanced care planning,provided by‌ a team of medical directors,advanced ​practice providers,registered nurses,and community health workers.
