Shoulder pain is a common complaint that can stem from various causes, but one frequent culprit involves the group of muscles and tendons known as the rotator cuff. This structure plays a vital role in stabilizing the shoulder joint and enabling a wide range of arm movements, including lifting and rotating the arm away from the body. When these tendons become irritated, inflamed, or torn, it can lead to significant discomfort and functional limitations.

The rotator cuff consists of four muscles—the supraspinatus, infraspinatus, subscapularis and teres minor—each connected to the humeral head by tendons. These tendons help keep the ball of the upper arm bone securely within the shoulder blade socket while facilitating movement. Problems arise when part of this system becomes damaged due to overuse, injury, or degenerative changes associated with aging.

Symptoms of rotator cuff issues often include shoulder pain, particularly when lifting the arm or lying on the affected side, along with weakness and reduced range of motion. Individuals may find it difficult to perform routine activities such as combing their hair or reaching behind their back. Pain may be described as a dull ache deep in the shoulder and can worsen at night, interfering with sleep.

Rotator cuff tendinitis refers to irritation and inflammation of the tendons, sometimes accompanied by inflammation of the bursa—a fluid-filled sac that reduces friction between tissues. This condition is also known as impingement syndrome and can result from repetitive overhead motions, prolonged arm positioning, poor posture, or participation in sports like tennis, baseball, swimming, or weightlifting. Over time, chronic tendinitis may contribute to tendon wear and increase the risk of tearing.

A rotator cuff tear occurs when one of the tendons detaches from the bone, either partially or completely. Tears can develop suddenly from a fall or abrupt lifting motion, or they may progress gradually due to long-term degeneration. Acute tears are often linked to trauma, while chronic tears typically emerge in individuals with a history of tendinitis or impingement, especially as they age.

Diagnosis involves a clinical evaluation of symptoms, strength, and range of motion, often supplemented by imaging tests such as ultrasound or magnetic resonance imaging (MRI). These tools help visualize soft tissue damage and determine the extent of tendon involvement. Early detection allows for timely intervention, which may prevent further deterioration.

Treatment approaches depend on the severity and type of injury. Conservative measures include rest, activity modification, physical therapy to strengthen surrounding muscles and improve flexibility, and over-the-counter pain relievers to manage discomfort. In cases where symptoms persist despite non-surgical care, or when a tear is large and significantly impairs function, surgical repair may be considered to reattach the tendon to the bone.

Preventive strategies focus on maintaining shoulder strength and flexibility through regular exercise, practicing good posture, avoiding prolonged repetitive overhead activities, and using proper technique during sports or manual labor. Addressing early signs of discomfort can help reduce the likelihood of progression to more serious injury.

While rotator cuff problems become more prevalent with age, particularly in individuals over 40, they are not exclusive to older adults. Younger people engaged in repetitive shoulder use or those who experience acute trauma are also at risk. Recognizing symptoms early and seeking appropriate medical evaluation can improve outcomes and support a return to normal activity levels.
