Menopause, a natural transition in a woman’s life, is increasingly recognized not just for its physical effects, but also for its impact on brain health. This period is often accompanied by symptoms like hot flashes, sleep disturbances and mood swings, but emerging research highlights significant alterations in brain function that can affect cognition, mental wellbeing, and potentially long-term neurological risk.

The average age of menopause is around 49 years old, and while many women seek relief from symptoms, understanding the brain’s role in this transition is crucial for developing strategies to support cognitive health throughout a woman’s life. Researchers are now analyzing large datasets to pinpoint the specific ways menopause affects the brain.

Menopause and Changes in the Brain

A recent analysis of data from nearly 125,000 women participating in the UK Biobank – a large-scale biomedical database – revealed associations between menopause and both mental wellbeing and brain structure. Participants were categorized as pre-menopausal, post-menopausal, or post-menopausal and using hormone replacement therapy (HRT).

The study found that post-menopausal women were more likely than pre-menopausal women to report symptoms of anxiety and depression. They were also more likely to seek help from a doctor or psychiatrist and to be prescribed antidepressants. Sleep disturbances, including insomnia, shorter sleep duration, and increased fatigue, were also more common after menopause.

Brain imaging analyses revealed significant reductions in grey matter volume following menopause. Grey matter, a key component of the central nervous system composed mainly of brain cells, showed the most pronounced reductions in regions critical for learning and memory – specifically the hippocampus and entorhinal cortex – and areas involved in emotional regulation and attention, known as the anterior cingulate cortex.

Notably, the hippocampus and entorhinal cortex are among the earliest brain regions affected in Alzheimer’s disease, the most common form of dementia. These findings suggest that menopause-related brain changes may contribute to an increased vulnerability to Alzheimer’s disease later in life, potentially explaining the higher prevalence of dementia observed in women.

Hormone Replacement Therapy and Brain Health

Hormone replacement therapy (HRT) is a common intervention for managing menopausal symptoms, but its effects on brain health remain a complex and debated topic. Approximately 15% of women in England are prescribed HRT, with rates varying across Europe, reaching as high as 55% in France.

The UK Biobank study investigated whether taking HRT post-menopause had any effect on these health outcomes. Interestingly, the research did not find that HRT improved the reduction in brain grey matter. Women using HRT showed higher levels of anxiety and depression compared to post-menopausal women who had never used HRT. However, further analysis suggested that pre-existing mental health problems may have influenced the decision to begin using HRT, rather than the medication itself causing these symptoms.

One potential benefit of HRT was observed in cognitive performance, specifically psychomotor speed. Psychomotor slowing is a hallmark feature of aging. Post-menopausal women who had never used HRT showed slower reaction times compared with both pre-menopausal women and post-menopausal women who had used HRT, indicating that HRT may help slow menopause-related declines in psychomotor speed.

However, the effectiveness of HRT can vary. Recent data suggests that as many as one in four women using the highest licensed dose of HRT still have low levels of estradiol (a form of estrogen), around 200 picomoles per liter. Older women and those using HRT patches were more likely to have lower estradiol levels. Optimal plasma levels to relieve menopause symptoms are between 220-550 picomoles per liter, suggesting that a significant proportion of women may not be receiving a fully effective dose.

Lifestyle Factors and Brain Health During Menopause

While research into HRT continues, there is growing evidence that healthy lifestyle habits can play a significant role in mitigating menopause-related changes in brain health. Regular exercise, engaging in cognitively stimulating activities like learning a new language or playing chess, maintaining a nutritious and balanced diet, prioritizing good sleep, and fostering strong social connections are all associated with improved brain health, cognitive reserve, and resilience to stress.

Research also demonstrates that regular physical activity can increase the size of the hippocampus, potentially helping to offset some of the menopause-related reductions observed in this region. Prioritizing sleep is also critically important, as it supports memory consolidation and helps clear toxic waste byproducts from the brain, processes essential for brain health and immune function.

More research is needed to fully understand the complex interplay between menopause, HRT, and brain health. Resolving the question of whether HRT is beneficial – including its potential to prevent brain grey matter volume reductions and reduce the risk of dementia – remains a priority. Determining the optimal dose and route of administration for HRT is also crucial. In the meantime, adopting a healthy lifestyle may offer an accessible and effective strategy to promote brain health during and after the menopause transition.
