Metformin and Metal Metabolism: A New Understanding
Metformin, a first-line medication for type 2 diabetes, has long been recognized for its ability to improve glucose control. Though, the precise mechanisms behind its action have remained incompletely understood.new research from Kobe University in Japan suggests a key role for metal metabolism. Researchers compared blood serum levels of various metals in patients who took metformin versus those who did not.
Published in BMJ Open Diabetes Research & Care, the study provides the first clinical evidence of altered blood metal levels in patients taking metformin. The team found that patients on metformin had significantly lower levels of copper and iron,and higher levels of zinc.”It is significant that we could show this in humans,” stated Dr. Ogawa, lead researcher on the project.
Dr. Ogawa further explained that the observed changes are potentially beneficial: “Decreases in copper and iron concentrations and an increase in zinc concentration are all considered to be associated with improved glucose tolerance and prevention of complications, these changes may indeed be related to metformin’s action.”
Imeglimin: A New Drug with a Different Approach?
Japan recently approved imeglimin, a novel diabetes drug derived from metformin. Though, imeglimin is designed to avoid the same metal-binding properties as metformin. Researchers are actively investigating the differences between the two drugs.
“Imeglimin is thought to have a different method of action, and we are already conducting studies to compare the effects the two drugs have,” said Dr. Ogawa. This comparative research aims to determine whether the metal-altering effects of metformin are crucial to its therapeutic benefits, or if imeglimin can achieve similar results through a different pathway.
The Bigger Picture: Towards Metal-Based Diabetes Therapies
The Kobe University team emphasizes that understanding the link between metformin,metal levels,and diabetes is about more than just refining existing treatments. They envision a future where manipulating metal concentrations in the body could lead to entirely new therapeutic strategies.
“We need both clinical trials and animal experiments to pinpoint the causal relationship between the drug’s action and its effects,” Dr. Ogawa explained. “if such studies progress further, they may lead to the growth of new drugs for diabetes and its complications by properly adjusting the metal concentrations in the body.”

This research was supported by grants from the Japan Society for the Promotion of Science (grant 24H00638) and the Manpei Suzuki Diabetes Foundation. The study was conducted in collaboration with a researcher from the Kagayaki Diabetes and Endocrinology Clinic Sannomiya.
