Minimally invasive glaucoma surgeries that act on Schlemm’s canal offer little to no difference in keeping patients medication-free compared to conventional procedures, according to recent medical evidence evaluated through October 2025. Open-angle glaucoma, a chronic eye condition driven by ineffective fluid drainage, damages the optic nerve and progressively impacts peripheral vision. While standard interventions like trabeculectomy bypass drainage channels entirely, newer canal-based minimally invasive glaucoma surgery, known as SC-MIGS, targets the eye’s natural drainage network to reduce intraocular pressure with fewer complications.

Clinical Outcomes of SC-MIGS Versus Standard Glaucoma Surgery

Evaluating 10 studies involving 639 participants aged 41 to 74 across multiple global regions including Italy, Japan, India, the United States, Latin America, Poland, Canada, and China, researchers assessed the efficacy of SC-MIGS against alternative treatments. When researchers compared SC-MIGS directly with conventional glaucoma surgery across three studies comprising 99 people, they found no clear difference in the proportion of patients remaining medication-free, progressive visual field loss, average pressure changes, or post-surgical drop counts. However, outcomes varied when SC-MIGS was paired with other common procedures. In an evaluation of two studies covering 132 patients, individuals who underwent SC-MIGS combined with cataract surgery were more likely to remain medication-free in the short term and experienced greater reductions in eye pressure compared to those receiving cataract surgery alone. That combined benefit carried a trade-off, as data showed a higher risk of bleeding for patients receiving the dual procedure.

Comparisons With Laser Treatments and Other MIGS Procedures

When measured against laser treatments in a single study of 30 participants, SC-MIGS reduced eye pressure more effectively, though it similarly carried an increased risk of bleeding. In contrast, when researchers compared SC-MIGS against types of minimally invasive glaucoma surgery that do not act on Schlemm’s canal across two studies involving 238 people, results indicated no significant differences in medication independence, post-operative drop counts, or the subsequent need for additional surgeries. Furthermore, a comparison between different types of SC-MIGS across two studies totaling 140 participants showed no substantial performance divergence among the canal-based devices themselves. Standard medical management for open-angle glaucoma typically relies on eye drops, laser procedures, or incisional surgeries like a trabeculectomy when conservative methods fail to control pressure. Normal eye pressure ranges between 10 mmHg and 20 mmHg, though damage can occur within or outside these parameters. While conventional filtering procedures successfully lower pressure by cutting into the eye surface, they carry notable risks including very low eye pressure or bleeding. Canal-based procedures were developed to preserve tissue structure, yet evidence regarding their long-term superiority remains constrained by limited study sizes and methodological variations across specific patient demographics.
