The trial results mark a significant milestone for individualized neoantigen therapies and mRNA platforms in oncology, demonstrating statistically significant improvements in recurrence-free survival when combined with the immunotherapy drug Keytruda.

Phase 3 Results and Clinical Trial Design

The clinical trial, designated INTerpath-001, evaluated 1,137 patients with stage IIB to IV cutaneous melanoma that had been completely surgically removed, according to Mexconomy. Patients received either the intismeran autogene vaccine in combination with Merck’s Keytruda, also known as pembrolizumab, or Keytruda alone. The treatment functions as an adjuvant therapy designed to help the immune system detect residual cancer cells following surgery. Both companies plan to present the complete data set at an upcoming international medical conference and initiate discussions with regulatory agencies regarding potential approvals.

How Personalized mRNA Neoantigen Vaccines Work

Unlike traditional preventive vaccines, intismeran autogene is not designed to stop healthy individuals from developing skin cancer. Researchers then select tumor signals known as neoantigens to produce a synthetic mRNA molecule capable of encoding up to 34 distinct targets. When administered, this genetic blueprint instructs cells to temporarily display the selected neoantigens, prompting the immune system to generate specific T-cell responses against cells carrying the tumor’s unique mutations. Mexconomy reports that this approach pairs the vaccine with Keytruda, an immunotherapy that releases the brakes on the immune system, creating a dual-action strategy that attacks the cancer from two fronts.

Market Impact and Expansion Into Other Cancers

The positive Phase 3 data generated strong financial reactions across the biotechnology sector. Mexconomy reported that Moderna shares surged 87 percent in pre-market trading following the announcement, while Merck shares rose 7 percent. Other vaccine manufacturers also gained ground, with BioNTech rising 7.5 percent and Novavax increasing by 4 percent. Building on these results, Merck and Moderna are evaluating intismeran autogene across nine ongoing Phase 2 and Phase 3 clinical trials targeting other oncological indications. According to Mexconomy, these expansion studies include non-small cell lung cancer, bladder cancer, and renal cell carcinoma, signaling a broader push to test personalized mRNA platforms across multiple tumor types.
