The Rising Tide of Neuroimmune Research
A recent forum held at Merck Research Laboratories in Boston brought together leading scientists, industry executives, and early-career researchers to explore the increasingly vital role of neuroimmune mechanisms in drug discovery. The ELRIG neuroimmunology forum, a key event in the field, focused on how immune cells within the brain – microglia, astrocytes, oligodendrocytes, and infiltrating immune cells – contribute to devastating neurodegenerative and neuroinflammatory diseases like Alzheimer’s, Parkinson’s, and multiple sclerosis.

Spotlight on Neuroinflammation and “Inflammaging”
A central theme of the forum was the growing recognition of neuroinflammation and “inflammaging” – the gradual accumulation of chronic, low-grade inflammation with age – as key drivers of neurodegeneration. Swarna Balasubramanian, Director of Search & Evaluation at AstraZeneca and a forum co-director, emphasized the urgency of focusing research on these areas. “The importance of neuroinflammation, and also inflammaging, are the two main things in the context of neurodegeneration,” she stated.
Inflammaging, in particular, is gaining attention as a subtle but powerful contributor to age-related cognitive decline. This persistent, low-level inflammation can disrupt cellular health over decades, frequently enough going unnoticed until critically important damage has occurred. Balasubramanian noted a shift in understanding Alzheimer’s disease, moving away from a simple association with aging towards a more nuanced recognition of its specific biological mechanisms, enabling earlier detection and driving clinical trials.
The pursuit of a “healthier lifespan” – extending not just longevity but also quality of life – is becoming a key focus, with startups exploring strategies to target inflammaging and improve vaccine response in older populations.
Rethinking the Role of Glial Cells
the forum also highlighted a paradigm shift in our understanding of glial cells – traditionally viewed as merely supportive elements in the brain. Matt Fell, Executive Director of Neuroimmunology at Merck and the forum’s other co-director, explained, “Someone who started out 25 years ago… back then, everyone would talk about glia, ‘what do they do? They just hold the brain together.’ Now we’re understanding these cells are not just innocent bystanders – they’re actively playing a role in disease and in the progression of the disease.”
This evolving understanding is fueled by growing evidence that the central nervous system isn’t isolated from the body’s immune system, opening up new avenues for therapeutic intervention.Fell suggested that inflammatory processes may begin earlier in life than previously thoght, perhaps taking root in midlife as protein clearance mechanisms decline and the immune system becomes overactive.
Funding Concerns and the Research Ecosystem
A critical discussion point was the precarious state of US research funding. Fell warned that cuts to academic funding, particularly in the Boston/Cambridge biotechnology hub, could have a detrimental “knock-on effect” on the entire innovation ecosystem. He emphasized the vital role academic institutions play in early-stage target validation, basic biology research, and training the next generation of scientists.A recent “pharma fly-in” to Massachusetts legislators underscored the industry’s concerns about the downstream consequences of these funding reductions.
