Inquest into Shane O’Connor’s Death
Shane O’Connor, the son of singer Sinéad O’Connor, disappeared from Tallaght University Hospital (TUH) on January 6, 2022. He was a high-risk patient under 24/7 care after a drug overdose. Less than a day later, his body was found near the Corke Abbey housing estate in Bray, Co Wicklow.
The inquest revealed that Shane went missing shortly after a nurse prevented him from going outside for a cigarette. At the time, there was no healthcare assistant available to provide the necessary supervision. Shane had been admitted to the hospital on December 29, 2021, after a severe drug overdose that damaged his liver. His condition improved, but medical staff remained concerned about his mental health.
Dr. Anthony O’Connor, who oversaw Shane’s treatment, expressed regret for the lack of supervision due to Covid-19 staff shortages. He acknowledged that Shane’s pleasant behavior led him to underestimate the risk of Shane leaving the hospital. Dr. Paula McKay noted that the need for constant supervision was recognized, but it was not consistently available.
The inquest highlighted that Shane had a troubled past, having been under the care of Tusla and living in a care facility before staying with his mother. Sinéad O’Connor identified her son’s body at the mortuary on January 8, 2022. Significant family members attended the inquest, including Shane’s grandfather Seán O’Connor and his father, musician Dónal Lunny.
Evidence showed that Shane had been admitted to psychiatric units multiple times due to misuse of cannabis and other behavioral issues. Dr. Mary Rose McCarthy indicated Shane’s struggles with support services and reported his mother felt overwhelmed by his condition.
On January 7, 2022, a contractor discovered Shane’s body. His last sighting was captured on CCTV at a DIY store in Bray on January 6. A receipt, a can of Coke, and sweets were found with Shane. He was pronounced dead at 11:15 AM on January 7.
