The study, described as an “umbrella review,” wasn’t a new clinical trial itself, but rather a synthesis of existing research. Researchers pooled data from several previous systematic reviews, encompassing dozens of recent trials and involving over 12,000 patients experiencing knee, hip, or hand osteoarthritis. This approach provides a broader perspective than any single study could offer.
When comparing exercise to doing nothing or a placebo, the review found that exercise resulted in a small reduction in pain – between 6 and 12 points on a 100-point scale – in the hip, knee, and hand. However, it didn’t demonstrate a significant improvement in physical function compared to these control groups.
Interestingly, the review also found that exercise offered pain relief comparable to common medications like ibuprofen or corticosteroid injections. However, it was less effective than a total joint replacement surgery.
Despite the value of this review, it’s important to acknowledge its limitations. A key issue is that all types of exercise were grouped together – including strength training, aerobic exercise, stretching, aquatic exercise, and Tai Chi. Previous research suggests that different types of exercise may have varying degrees of effectiveness. The study also didn’t differentiate between exercise performed under the guidance of a professional and exercise done independently at home. Supervised training often yields better results. The duration of the studies included in the review was relatively short – around 12 weeks in most cases. The benefits of exercise often accumulate over longer periods of consistent participation. The intensity and weekly dosage of exercise weren’t consistently accounted for either, and evidence suggests that around 150 minutes of moderate-intensity activity per week may be optimal.

Regular walking is an excellent option, particularly for those who enjoy outdoor activities. Strength training is also beneficial for supporting the muscles surrounding the joint. Intensity can be gradually increased as tolerated, aiming for a level where conversation becomes slightly more challenging.
