Patients diagnosed with pancreatic cysts face a risk of cancer progression that increases sharply as the number of worrisome features grows, according to a prospective study published on August 20, 2026, in the journal Gastroenterology. Conducted by researchers at Mayo Clinic, the study estimates the three-year risk of progression to pancreatic cancer or advanced precancerous changes based on specific imaging traits, providing a clearer benchmark for medical surveillance and surgical decisions.
Three-Year Cancer Risk Tied to Cyst Features
The study followed 230 participants with pancreatic cysts that displayed either worrisome or high-risk features. Researchers tracked these patients over time regardless of whether they chose immediate surgery or ongoing clinical surveillance, according to findings detailed by Mayo Clinic. Among the participants, 32 underwent surgery within the first three months, while 198 entered clinical monitoring. Of those monitored, 185 had cysts with worrisome features and 13 had cysts with high-risk traits.
The risk of developing pancreatic cancer or advanced precancerous lesions within three years scaled directly with the count of worrisome features identified on imaging. Patients with a single worrisome feature faced about a 2% chance of progression. That risk doubled to 4% for individuals with two worrisome features, and jumped steeply to 18% for those presenting with three or more worrisome features.
“This information will help us counsel patients,” Shounak Majumder, lead author and a gastroenterologist at Mayo Clinic, said in a statement. “If we want to intercept pancreatic cancer while also sparing patients from unnecessary interventions, we must really know what is likely to become cancer and what is not.”
Balancing Surveillance and Surgery
Pancreatic cysts are frequently discovered incidentally during imaging scans performed for unrelated medical reasons. While most cysts never become malignant, physicians classify them based on imaging characteristics into categories of high-risk features, worrisome features, or neither. Cysts without worrisome or high-risk traits carry a relatively low likelihood of progression. Conversely, high-risk features often prompt doctors to recommend surgery due to heightened cancer concerns.
For cysts with worrisome features, physicians often recommend close monitoring, though some cases still lead to surgical removal. Dr. Majumder noted that uncertainty surrounding whether a specific cyst will progress creates significant anxiety for patients undergoing surveillance, occasionally driving them to choose surgery. More precise three-year risk estimates aim to help physicians and patients weigh the benefits of continued monitoring against surgical risks.
Furthermore, the study observed that pancreatic cancers diagnosed among patients enrolled in careful surveillance programs were detected at very early stages. This outcome suggests that regular monitoring of appropriately risk-stratified patients provides a viable opportunity to catch the disease early, making surveillance an effective alternative to immediate surgery for specific groups.
