What is ⁤Syphilis and Why is Treatment ​Challenging?

Syphilis is a sexually transmitted bacterial infection caused ⁤by Treponema pallidum. It’s spread through direct contact with a syphilitic sore, ⁢known as a chancre,⁢ typically during vaginal, anal, or oral sex. The Centers for Disease Control and Prevention (CDC) reports a notable increase in syphilis cases in⁢ recent years, with a notably alarming rise in congenital syphilis-syphilis passed from⁣ a pregnant person to ‌their baby according to CDC data released in October 2023.

Untreated syphilis can led to ‍serious health problems, including damage to the heart, brain, and‍ other organs. ​ Treatment with antibiotics, specifically penicillin, is highly effective, but access to the standard treatment, benzathine penicillin G (BPG), ‍has been repeatedly disrupted by drug shortages.

The BPG Shortage: A Recurring Crisis

Benzathine penicillin G, marketed as Bicillin L-A, is the ‌recommended⁤ treatment for early-stage syphilis. Though,the United States ⁢has experienced numerous shortages of BPG over the past two decades,including ​a particularly prolonged shortage lasting nearly five years. The Food ​and Drug⁢ Administration⁢ (FDA) maintains a list of current⁢ and resolved drug shortages, highlighting the ongoing vulnerability of the pharmaceutical ⁣supply chain.

Most recently, in July 2025, Pfizer recalled several lots of Bicillin L-A due to potential ⁣particulate contamination, exacerbating‍ an existing shortage that began‍ two years prior. To address the shortfall,the U.S. is⁣ currently importing‍ the medication from other countries, ⁣a temporary solution with logistical and cost implications. Pfizer announced the⁣ recall‍ on July 18, 2025.
