A new, potentially game-changing approach to cervical cancer screening is showing promise: testing menstrual blood for signs of the human papillomavirus (HPV). A study published in the BMJ suggests that analyzing a sample collected via a sanitary pad could be a convenient, non-invasive, and accurate alternative to traditional screening methods.

Currently, cervical cancer screening typically involves a clinician collecting a sample of cells from the cervix. While effective, this process can be a barrier for many women. Approximately one-third of women invited for screening do not participate, often due to fear, discomfort, or logistical challenges. This new method aims to address those barriers by allowing women to collect a sample themselves, at home.

The study, conducted in Hubei, China, between 2021 and 2025, involved over 3,000 women aged 20 to 54 with regular menstrual cycles. Researchers compared the accuracy of HPV detection in menstrual blood samples – collected using a sanitary pad and a specialized strip – to samples collected directly by clinicians. Participants provided three samples: one from menstrual blood, one collected by a clinician, and another processed in a lab by a health worker.

The results were encouraging. The menstrual blood test demonstrated a sensitivity of 94.7% for detecting CIN2, a type of cervical cell abnormality that may require treatment. This represents comparable to the 92.1% sensitivity achieved with clinician-collected samples. While the specificity – the ability to correctly identify those without the disease – was slightly lower, the overall probability of a negative test result being accurate was the same for both methods. Importantly, the number of referrals for further testing was also similar between the two approaches.

“The results of this large-scale, community-based study show the utility of using minipad-collected menstrual blood for HPV testing as a standardised, non-invasive alternative or replacement for cervical cancer screening,” the study authors concluded.

The significance of this research extends beyond convenience. Cervical cancer is largely preventable through screening and vaccination against HPV. However, access to screening remains uneven, particularly among younger women, those with disabilities, women from ethnic minority communities, and members of the LGBTQ+ community. Offering a more accessible and comfortable testing option could significantly improve participation rates and ultimately reduce the incidence of cervical cancer.

Athena Lamnisos, chief executive of the Eve Appeal, a gynaecological cancer charity, called the findings “exciting.” “It’s exciting to see new, more acceptable and potentially gentler ways of offering what could be a life-saving test to prevent cervical cancer from developing,” she said.

However, experts caution that this research is still in its early stages. Sophie Brooks, health information manager at Cancer Research UK, emphasized the need for further trials involving larger and more diverse populations. “Testing menstrual blood for HPV is an interesting, non-invasive approach, and could potentially offer another option in the future,” she stated, “But it was early days, and trials involving larger and more diverse groups would be required to understand how well it worked for different people, and whether it could fit into existing screening pathways.”

Xavier Bosch, an emeritus researcher at the Catalan Institute of Oncology, who was not involved in the study, described the work as “very pioneering” but stressed that its clinical applications remain unclear at this time.

It’s also important to note that this method isn’t universally applicable. As Lamnisos pointed out, women who have gone through menopause, and therefore no longer menstruate, would not be able to utilize this testing method. The test relies on the presence of menstrual blood, meaning it wouldn’t be suitable for everyone.

The National Health Service (NHS) in the UK is already exploring at-home screening options, including DIY test kits containing vaginal swabs, for women who have missed several scheduled appointments. The introduction of menstrual blood testing could potentially expand these options further, offering women a choice of methods that best suit their individual needs and preferences.

While more research is needed, the prospect of a convenient, at-home test for HPV detection represents a significant step forward in cervical cancer prevention. The ability to offer multiple screening options could empower more women to take control of their health and reduce the burden of this preventable disease. The focus remains on ensuring equitable access to effective screening methods for all those at risk.
