Coordinated Care Improves Patient Outcomes and Efficiency
A collaborative initiative between the Cantonal health Office (OCS), Geneva University Hospitals (HUG), and the Geneva Home Supervisory Institution (IMAD) has yielded notable positive results in post-surgical rehabilitation. The “RAC Network” program, launched with funding from the Canton of Geneva, exemplifies a commitment to strengthening coordination across the healthcare system for the benefit of residents.
The program focuses on early, personalized contact between hospital and home-based care providers, leading to improved quality of care, reduced postoperative complications, and optimized resource utilization. State advisor Pierre Maudet highlighted the program as a demonstration of Geneva’s vision for an integrated, accessible, and patient-centered healthcare system.
Three-Phase Approach Reduces Hospital Stays and Costs
The RAC Network targets fragile patients and those requiring additional support before and after surgery.it employs a three-phase approach: prehabilitation at home, hospitalization, and coordinated rehabilitation in the home.This integrated pathway has demonstrably reduced hospital stays, considerably lowered readmission rates, and achieved a cost savings of 17.8% on overall care expenses.
Expanding Program Scope to Benefit More Patients
Initially implemented for intestinal, lung, and hip surgeries, the program has successfully expanded to include liver and pancreas procedures. Further expansion to esophageal surgeries and other organ-specific procedures is planned. this expansion positions the RAC Network as a key component of a sustainable, extensive healthcare system focused on patient needs.
