Pharmacists are uniquely positioned to guide patients toward dietary changes that can significantly improve cardiometabolic health, even without drastic overhauls. That’s according to Hana Kahleova, MD, PhD, MBA, director of clinical research at the Physicians Committee for Responsible Medicine, who discussed practical strategies for integrating plant-based dietary guidance into routine care for patients with obesity, diabetes, and dyslipidemia.

Dr. Kahleova emphasizes that meaningful improvements in health markers can be achieved through low-fat, plant-based eating patterns without the need for calorie counting or carbohydrate restriction. This approach can be particularly beneficial for individuals struggling with glycemic control or weight management. The key, she suggests, isn’t to demand a complete dietary transformation, but to identify plant-based foods patients already consume and encourage small, incremental substitutions.

“Our clinical research studies show that a low-fat vegan diet can help improve glycemic control and lead to significant weight loss, without requiring calorie counting or carbohydrate restriction,” Dr. Kahleova explained. “Participants are encouraged to eat freely from plant foods while avoiding animal products and limiting added fats. In routine pharmacy practice, the goal does not need to be full dietary transformation. Pharmacists can start by helping patients recognize plant-based meals they already eat—such as oatmeal, toast with jam, bean burritos, vegetable soups, pasta with marinara sauce, or rice and beans.”

This approach of building on existing habits makes change feel more achievable. Pharmacists can then suggest one or two simple substitutions per week, such as replacing meat in a favorite recipe or choosing a plant-based option when dining out. Small, consistent changes, Dr. Kahleova argues, are often more sustainable than complete overhauls.

However, adopting a more plant-based diet isn’t always straightforward. Common barriers include concerns about taste, cost, convenience, and family acceptance. Dr. Kahleova acknowledges these concerns and offers practical strategies for addressing them. She reassures patients that plant-based eating doesn’t necessarily require unfamiliar foods or extensive cooking. Many familiar foods—pasta, sandwiches, cereals, soups, and stir-fries—can easily be adapted to be plant-based.

Involving family members in meal planning and exploring restaurants with plant-based options can also improve adherence. Community support, through local or online plant-based groups, cooking classes, or educational resources, can provide additional guidance and encouragement.

For patients already managing their conditions with medication, pharmacists can frame dietary changes as a complement—not a replacement—for pharmacotherapy. Research indicates that adopting a low-fat, plant-based diet can significantly improve cholesterol levels, glycemic control, and body weight, potentially allowing for medication dose adjustments under a physician’s supervision.

“Pharmacists can explain that medications treat the symptoms of cardiometabolic disease, while diet addresses many of the underlying causes—such as excess saturated fat intake, hepatic fat accumulation, and insulin resistance,” Dr. Kahleova stated. “By reinforcing that dietary improvements may enhance medication effectiveness and potentially reduce long-term medication burden, pharmacists can motivate patients while emphasizing the importance of ongoing monitoring and collaboration with their healthcare team.”

A recent meta-analysis published in JAMA Network Open supports this integrated approach. The review of 29 randomized controlled trials found that vegetarian and vegan diets led to greater improvements in cholesterol, blood sugar, and body weight compared to standard therapy for cardiovascular disease. 1 This suggests that dietary interventions can play a crucial role in optimizing patient outcomes alongside traditional medical treatments.

a systematic review of evidence published in Cureus in February 2025 synthesized findings from 32 longitudinal studies, highlighting the potential of plant-based diets in reducing the risk of chronic diseases such as type 2 diabetes, cardiovascular disease, obesity, and metabolic syndrome. 2

Dr. Kahleova also points out an important nuance in the broader discussion around processed foods. She emphasizes that public health conversations should differentiate between ultra-processed animal foods—particularly processed meats and sweetened beverages—which are strongly linked to adverse cardiometabolic outcomes, and ultra-processed plant-based foods, which are often associated with neutral or even beneficial effects.

“For pharmacists, this distinction is empowering,” Dr. Kahleova concludes. “It allows for practical, patient-centered counseling that prioritizes achievable dietary improvements rather than rigid food rules—supporting better long-term adherence and better cardiometabolic health.”
